About this file
This document is a Data Breach Report Form (Exhibit G) associated with Contract CTR066192 - Amendment 4, issued by the Arizona Department of Economic Security (ADES). The form provides a standardized template for organizations to report data breaches, with specific instructions for submission within 24 hours of an incident to the contract's Program Specialist.
The form requires detailed documentation of a data breach, including the date of occurrence, date of notification, a description of the incident, the organization's response, and proposed preventative measures for future incidents. Notably, the form emphasizes protecting Personally Identifying Information (PII) by instructing users to avoid disclosing specific personal details and instead use generic terms like "Survivor" or "client" when describing the breach. The document is designed to ensure transparent and rapid communication about potential data security incidents while maintaining confidentiality.
