About this file

This is a Contractor Responsibility Form issued by the City of Tucson, Arizona for the Stone Avenue Youth Transitional Housing Project (Solicitation Number 250133). The form must be completed and submitted by the Apparent Low Bidder within four business days following bid opening and notification of low bidder status. The project involves exterior and interior improvements to an Econolodge building containing 50 rooms, which will be remodeled from single rooms into studio kitchenettes. Major building systems including electrical, HVAC, plumbing, and structural components will be replaced or modified to meet current code requirements and safety standards. The exterior will include landscape improvements incorporating green stormwater infrastructure (GSI) practices for water harvesting and community gathering and gardening spaces. Upon completion, the facility will serve as low-income transitional housing for youth.

The form requires contractors to certify their responsibility through multiple criteria, including adequate financial resources, ability to meet performance schedules, satisfactory performance records, demonstrated integrity and ethics, adequate experience, technical expertise, quality controls, and access to necessary equipment. Contractors must disclose any debarment, suspension, or ineligibility; criminal convictions or civil judgments related to public contracts within the prior three years; and provide details on liquidated damages, project terminations, claims against bonds, and pending legal actions. Applicants must submit current Experience Modification (EMod) ratings, describe three representative similar projects with owner references and original versus actual construction schedules, list current workload and resource availability, and acknowledge that failure to provide sufficient information may result in a non-responsibility determination and contract termination if any statements are later found to be erroneous.
