About this file

This document is a Contractor Affidavit form required by Section 787.06(13) of Florida Statutes for nongovernmental entities executing, renewing, or extending contracts with governmental entities, specifically the School Board of Manatee County, Florida. The affidavit is a mandatory certification that the contractor does not engage in any form of labor or service coercion, including prohibitions against using physical force, threatening isolation, establishing exploitative debt, confiscating identification documents, causing financial harm, enticing by fraud, or providing controlled substances for exploitation.

The affidavit requires the contractor's authorized representative to sign under penalty of perjury, attesting that their company will not use any of the specified coercive practices. The form provides blank fields for the contractor's name, address, contact information, and authorized representative details, allowing for individualized completion by each potential contractor. While directly linked to the Construction Management at Risk project for High School AAA in Manatee County, this document serves as a standard anti-coercion compliance form to ensure ethical labor practices across governmental contracting.
