About this file

This document is Amendment No. 5 to AHCA Contract No. EXD074, an agreement between the State of Florida, Agency for Health Care Administration (the "Agency") and Rutledge Ecenia (the "Vendor"). The amendment was executed in February 2020 to correct a previous amendment's reference and clarify the contract's renewal terms.

The amendment extends the contract period, which originally began on September 28, 2017, to now end on June 30, 2021. It exercises a final renewal option for a 12-month period beginning July 1, 2020, in accordance with Section 287.058 of Florida Statutes. The amendment allows for a contract renewal period that does not exceed three years or the original contract term, whichever is longer. All other provisions of the original contract remain unchanged, and the amendment is contingent upon the full execution of all previous amendments to the contract.
