About this file
This is a Task Assignment Change Order Form (TACO 19, Change Order No. 1) issued by the Florida Department of Environmental Protection (DEP) for Contract SW238. The contractor is Industrial Divers, Corp. Inc., represented by Rocco Galletta, with DEP Contract Manager Michell Smith overseeing the modification. The change order was dated March 29, 2021, and approved by Monica J. Brady (Budget Representative) and Teresa Booeshaghi (Contractual Authority).
The change order modifies Task 19 to extend the task completion date from March 31, 2021, to June 30, 2021 (a 90-day extension) due to extended equipment downtime, and increases the quantity of tires to be recovered from 15,000 to 16,495 units. The contract price is based on a retrieval cost of $18.02 per tire, resulting in a total task amount of $297,239.90 for the retrieval of 16,495 tires. The original task amount of $270,300.00 increased by $26,939.90 to reflect the additional 1,495 tires. Funding is allocated from Organizational Code 141132-20 under Object Code 37450104000 for fiscal year 2020, with the full amount of $297,239.90 charged to this budget line.
