About this file
This document is an Amendment No. 1 to DEP Contract No. HW554, a contract entered into on February 16, 2011, between the Florida Department of Environmental Protection (FDEP) and Shaw Environmental, Inc. The contract manager for FDEP is Teresa Booeshaghi. A Separation of Duties Notice accompanies this amendment, indicating that the contract requires adequate separation of duty in procurement and that the DEP Contract Manager must not execute the contract amendment themselves.
Amendment No. 1 decreases the funding increment amount of the contract from $2,080,000.00 to $1,955,000.00, representing a reduction of $125,000.00. The amendment adds new language requiring the contractor to certify that it is not listed on the Scrutinized Companies with Activities in Sudan List or the Scrutinized Companies with Activities in the Iran Petroleum Energy Sector List as created pursuant to section 215.473 of the Florida Statutes. The contractor further agrees that the Department may immediately terminate the contract if the contractor is found to have submitted false certification or if the contractor is placed on either of these lists during the contract term. All other terms, conditions, and attachments of the original contract remain in full force and effect. The amendment was executed on February 22, 2012, by Shaw Environmental, Inc., and on March 1, 2012, by the Florida Department of Environmental Protection.
