Contract Summary

Amendment No. 10 to Agency Term Contract No. GC790 is executed between the Department of Environmental Protection (DEP), an agency of the State of Florida, and AHS-GEAR LLC. The original contract for petroleum contamination site response action services became effective on April 9, 2014, and this amendment was signed on August 22, 2022, by John Truitt, Deputy Secretary of the Florida DEP, and James D. Hirsch, President of AHS-GEAR LLC.

This amendment modifies the Scope of Services, specifically Section 12 regarding Debris, Waste and Product Removal and Disposal under the Schedule of Pay Items. The revision establishes procedures for handling soil volume overages on purchase orders. If delivery weight tickets indicate an overage not exceeding 20% of the total volume authorized under a Purchase Order, the contractor is required to notify the site manager via a Request for Change within two business days of the additional soil being transported from the site. All other terms and conditions of the original contract remain in effect, with the amendment provisions controlling in the event of any inconsistency.
