DEP Contract No. GC655, Amendment No. 1
This is an Amendment No. 1 to a contract between the Florida Department of Environmental Protection (DEP) and Aerostar Environmental Services, Inc., executed on May 31, 2005. The contract was originally entered into between the State of Florida Department of Environmental Protection and Aerostar Environmental Services, Inc., headquartered at 11181 St. Johns Industrial Parkway North, Jacksonville, Florida 32346. The amendment was signed on April 13, 2006, and is managed by DEP Contract Manager Ferda Yilmaz. A Separation of Duties Notice was issued on March 21, 2006, indicating that adequate separation of authority is required for contract execution.
The contract provides petroleum contamination site cleanup services through an indefinite delivery vehicle using fixed rate, lump sum, cost reimbursement, and cost plus pricing methods. The contract period runs from May 31, 2005, to May 30, 2010. Amendment No. 1 updates rate schedules and procedures to align with the current Petroleum Preapproval Program Standard Operating Procedures Manual, specifically revising laboratory analysis turnaround times to three weeks maximum, updating drilling rates for mud rotary and hollow stem auger drilling up to 100 feet, and deleting outdated rate schedules. The amendment designates Scott Hughes as the Contractor's Co-Contract Manager. All references to rate schedules in task assignments shall reflect the versions of the Petroleum Preapproval Program Standard Operating Procedures Manual in effect at the time of task assignment execution.
