About this file

This document package comprises a Task Assignment Change Order for DEP Contract No. BS033 between the Florida Department of Environmental Protection (DEP) and Santa Cruz Construction, Inc. The change order, dated April 10, 2015, modifies Task Assignment Number 2 originally issued on November 14, 2014. The contract manager for Santa Cruz Construction is Kristopher Brown (321-452-3190), and the DEP contract manager is Marisa Rhian (863-534-7077). The document includes the original task assignment notification, the change order form, supporting email correspondence, and a detailed task description with maps of the treatment area.

The contract pertains to herbicide treatment of 64 acres of lead tree (Leucaena leucocephala) and Brazilian Pepper (Schinus terebrinthifolius) at the Tenoroc Fish Management Area in Lakeland, Florida. The original task amount was $17,380 with an April 16, 2015 completion date. Change Order No. 1 increased the task amount by $1,000 to $18,380 and extended the completion date by six weeks to May 28, 2015, due to a change in herbicide treatment methodology increasing foliar treatment hours. The revised cost breakdown includes $500 mobilization, $2,100 for cut tree and stump treatment (60 hours at $35/hour), $6,660 for foliar treatment (148 hours at $45/hour), and $9,120 for foreman/truck services (228 hours at $40/hour). The final inspection report and certification of 95% plant die-off must be submitted by May 26, 2015, with final invoice submission following DEP contract manager certification.

Can we decrease the cut stump hours by 20 and increase the foliar and foreman by hours by 20?

Also, we have 5g of unopened Habitat left from task 3.

Thanks

You are correct, there was a typo earlier:

Please send me a revised estimate if needed.

Can I let you know when we're done in the field today? I intended to ask for more foliar and fewer cut stump hours yesterday, but it slipped my mind. Also, I must have made a mistake in the billing hours. The foreman hours should be identical to the combined foliar and cut stump hours.

I'll get the task 4 estimate in today.

Kris, After review of the Task 2 invoice, I wanted to verify that you would be able to complete the task with the remaining hours.

Please let me know if this will be sufficient.
