About this file
This is Amendment 2 to RFQ C043779 for Medical Director and Physician Staffing services issued by the New York State Department of Health, Oxford Veterans Home, a 242-bed skilled nursing facility located in Chenango County, New York. The amendment modifies the original RFQ to require one Medical Director, no more than two Physicians, and two Advanced Practice Providers (APPs), all licensed in New York State. The Medical Director will provide on-site coverage 1-2 full working days per week, Monday through Friday, 8:00 a.m. to 4:30 p.m., with on-call availability for emergencies and regulatory inspections. Physician support staff will provide on-site coverage five days per week during the same hours with 24/7 on-call availability and a maximum 15-minute response time for urgent and emergent calls. Services include clinical leadership, oversight of medical policies and procedures, routine and as-needed resident evaluations, care coordination, electronic medical record documentation, and participation in care planning to ensure regulatory compliance. The RFQ deadline was August 13, 2026, at 4:00 p.m. ET, with written questions due July 24, 2026, and answers posted by July 31, 2026. This is a five-year contract with a veteran population serving primarily long-term stay residents with an expected census of 230 residents at 95% occupancy.
Cost proposals must be submitted as fully burdened fixed pricing for each contract year, including all labor, materials, overhead, administrative expenses, and travel. Bidders must base cost proposals on an estimated 60 residents annually with 70% or greater VA service-connected status, with reimbursement at fair market value based on a percentage of Medicare CPT Allowable Rates. Historical data indicates an average daily census of 217 of 242 beds, approximately 150 new admissions annually (13 per month), approximately 10 discharges per month, 80 after-hours calls per quarter, 90-110 falls per month, and an antipsychotic utilization rate of 10.8% among 25 residents. The Department reserves the right to award for Medical Director services only or for both services combined. Bidders must be medical practices or staffing agencies with a minimum of three years' experience placing or employing Medical Directors, Physicians, and APPs licensed in New York State; Medical Directors must have at least two years of medical administrative or supervisory experience, while other physicians and APPs must have at least one year of post-licensure experience in skilled nursing facilities or hospital-based long-term care units.

4 Scope of Work
The Department is seeking qualified vendors to provide a Medical Director and Physician Support Staff services for its skilled nursing facility. The selected vendor will be responsible for delivering comprehensive clinical oversight and medical services necessary to support the health, safety, and regulatory compliance of the resident population.
The Department requires one (1) Medical Director. Medical Director services are generally provided between the hours of 8:00 AM and 4:30 PM, Monday through Friday, with potential weekend days as determined by Department. Medical Directors are expected to be on-site at least 1-2 full working days per week. Medical Directors are also expected to be “on call” (outside of the 1-2 full working days per week) as needed, for emergencies or to address regulatory requirements, such as CMS/DOH/VA unannounced inspection surveys.
Services shall include, but not limited to, a combination of regularly scheduled onsite coverage, availability for on-call consultation, and timely response to urgent and emergent resident care needs. The Medical Director will provide clinical leadership, oversight of medical policies and procedures, and collaboration with interdisciplinary teams to ensure adherence to applicable federal and state regulations, including those governing skilled nursing facilities.
In addition to the Medical Director, the Department also requires: no more than two (2) Physicians, and two (2) Advanced Practice Providers (APPs). These positions are to provide services onsite at least five (5) days a week between the hours of 8:00 AM and 4:30 PM, Monday through Friday. They must also provide on-call medical coverage at all times (24 hours, 7 days a week) in the event there is no provider in the facility. Urgent and emergent calls must be responded to as soon as possible, no later than fifteen minutes from the time received.
Physician support staff services shall include, but are not limited to, routine and as-needed resident evaluations, coordination of care, documentation in the facility’s electronic medical record system, and participation in care planning processes. The vendor must ensure adequate staffing levels to meet the clinical demands of the Department, maintain continuity of care, and support quality outcomes.
The selected vendor will be expected to work collaboratively with Department administration, nursing leadership, and other healthcare professionals, while maintaining consistent communication, availability, and responsiveness across onsite and on-call services.
