About this file

This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees bidding on state contracts, specifically in connection with the Pennsylvania State Police Bureau of Criminal Investigation's solicitation for physical examination services for Clandestine Laboratory Investigations Team members. The certification form mandates compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and thirteen specific state labor and workforce safety laws, including provisions related to workplace misclassification, child labor, minimum wage, prevailing wage, equal pay, and healthcare overtime restrictions. Authorized contractor representatives must complete and submit the form with their bid responses no later than July 14, 2026 at 11:00 a.m., along with a completed cost matrix and current certificate of insurance listing the Pennsylvania State Police as certificate holder. All bid submissions must be made electronically through the DGS eMarketplace website and the Pennsylvania Supplier Portal.

The form serves as a binding certification of compliance with applicable Pennsylvania labor laws and establishes legal accountability for the contractor or grantee. False certifications carry significant penalties under Pennsylvania law, including contract termination and three-year ineligibility to bid on Commonwealth contracts under the debarment and suspension provisions of 62 Pa. C.S. § 531, as well as criminal liability under 18 Pa. C.S. § 4904 for unsworn falsification to authorities. Contractors must immediately report any changes in their compliance status to the purchasing agency. The certification requirement applies to all vendors seeking to provide services or products to the Commonwealth, regardless of contract value or scope.
