About this file
This document is an OSHA Respirator Medical Evaluation Questionnaire (Appendix C to 29 CFR 1910.134) that accompanies a Pennsylvania State Police (PSP), Bureau of Criminal Investigation (BCI), Clandestine Laboratory Response Team (CLRT) contract opportunity for member physical examinations. The questionnaire serves as a mandatory medical evaluation tool for employees selected to use any type of respirator equipment and must be completed by all such personnel. The contract requires vendors to provide initial and annual physical examinations for CLRT members on an as-needed basis. Interested vendors must submit electronic bids through the Pennsylvania Supplier Portal (www.pasupplierportal.state.pa.us) no later than July 14, 2026 at 11:00 a.m. Vendors must complete the cost matrix in its entirety, submit a completed Worker Protection and Investment form (BOP-2201), and provide current certificates of insurance listing PSP as the certificate holder with supplemental additional insured language.
The questionnaire itself is a comprehensive medical evaluation instrument divided into Part A (mandatory sections covering demographics, respirator type, and health history) and Part B (optional sections addressing occupational and environmental exposures). Part A requires all respirator users to provide personal information and answer questions regarding current health status, pulmonary and cardiovascular conditions, medications, and prior respirator use complications. Part B allows healthcare professionals discretion to inquire about high-altitude work, hazardous chemical exposures, occupational history, military service, current medications, respirator usage frequency, work intensity levels, environmental conditions, and specific toxic substance exposures. Questions regarding vision and hearing problems, back injuries, and musculoskeletal issues are mandatory only for employees selected to use full-facepiece respirators or self-contained breathing apparatus equipment. All vendors responding to this opportunity must be registered through the Pennsylvania Supplier Portal and are responsible for monitoring the DGS eMarketplace website for any changes or notices that will be incorporated into the bid requirements.
