About this file
This is a Bidder Requirements Checklist for the State of Florida's S-40 Erosion Repairs project (Bid #C007123). The checklist outlines mandatory qualifications and documentation that bidders must submit with their bid responses to be considered for award. All licenses, certifications, and qualifications must be active and current at the time of bid opening, and failure to meet any requirement results in automatic bid rejection.
Bidders must possess a current State of Florida General Contractor License and demonstrate a minimum of eight years of marine construction experience. Applicants are required to submit a staffing plan identifying key personnel and their relevant experience, with a Primary Contact/Project Manager having at least eight years of experience in the field. Bidders must provide a minimum of three client references for similar construction services performed within the past ten years, with relevant experience including diving in tannic water and low-visibility conditions, confined-space diving, grout injection, permanent sheet pile installation, and riparian restoration. At least one reference must demonstrate work completed in the Central Florida region. The contractor must employ a full-time Advanced TTC/MOT certified professional and a Certified FDEP Erosion Control Inspector (Tier 1 minimum), with flagging operations performed by personnel holding FDOT TTC Basic Flagger Certification. Additionally, bidders must designate a Dedicated Person in Charge (DPIC) with a minimum of ten years of commercial diving experience who will oversee and be present during all underwater work. Bidders must be pre-qualified in applicable FDOT Work Classes including Debris Removal, Grassing/Seeding/Sodding, Concrete Rehabilitation, Concrete Surface Sealer/Waterproofing, Driving Steel Sheet Pile, Grout Injection, Sealing, and Spall Repairs, or provide equivalent project experience demonstrating expertise. Upon bid award, the bidder must provide an updated employee list within thirty days and notify the owner of any staffing changes within three days.
