About this file
This document is a Bid Package Submittal Requirements guide issued by the Connecticut Department of Energy and Environmental Protection (CT DEEP) Engineering, Safety and Sustainability Division for the Construction of Squantz Cove Boat Launch project in New Fairfield, Connecticut. All bidders must submit their bid package documents electronically through the CT DAS CTSource website prior to the bid opening date and time. The submission requirements are divided into two categories: Table 1 lists mandatory documents required from all bidders prior to bid opening, including the Bid Proposal Form, Standard Bid Bond or Certified Check, General Contractor Bidder's Qualification Statement, DAS Prequalification Certificate, Certificate of Authority, Build America Buy America Acknowledgment Form if applicable, and Campaign Contribution Certification. Failure to submit any Table 1 documents by the bid opening deadline will result in automatic bid rejection and will not be considered a minor irregularity under Connecticut General Statutes 4b-95.
Table 2 specifies additional requirements for the Apparent Lowest Qualified Bidder, which must be submitted within ten calendar days after receipt of the Letter of Intent from the DEEP Engineering Unit. These documents include Named Subcontractor Bidder's Qualification Statements, DAS Prequalification Certificates and Update Statements for subcontractors with contracts exceeding $1,000,000, Good Faith Effort documentation filed with the Commission on Human Rights and Opportunities (CHRO), Contractors Wage Certification Form submitted to the CT Department of Labor, Project Agreement, Subcontract Agreement Forms, Certificate of Liability Insurance, Performance Bond, Labor and Material Bond, and surety documentation. Additional requirements apply to nonresident contractors, projects disturbing one or more acres of land requiring Stormwater Pollution Control Plan certification, and corporations requiring a Certificate of Legal Existence. Any delays in submitting Table 2 documents will extend the bid validity period by the same number of days the submission is delayed, with the DEEP Project Manager retaining sole discretion to reject late submissions.
