About this file
This is an Invitation for Bid (IFB) offer submission form for the State of Illinois, specifically related to Temporary Employee Services (IFB B-52949) administered by the Illinois Department of Corrections (IDOC). The document serves as the formal bid submission template that vendors must complete to respond to the state's solicitation for temporary employee services. The form requires bidders to submit offers in full compliance with all requirements outlined in the solicitation document, with failure to meet specified requirements resulting in disqualification. Bidders are instructed to reference Section A.1 of the solicitation for a complete list of required documents that must accompany their offer.
The form includes provisions for bidders to claim applicable preferences under the Illinois Procurement Code, including Illinois Business Preference and Domestic Product Preference, with vendors required to submit supporting documentation such as affidavits or product designations for qualifying items. Additionally, bidders may submit exceptions to the Standard Terms and Conditions by providing a redlined version of the contract terms; however, the State reserves the right to accept or reject any proposed exceptions. The Standard Terms and Conditions referenced in the solicitation are available on the Illinois Chief Procurement Office website. All completed bid forms must be signed by an authorized representative of the bidding company, certifying that the offer is submitted in accordance with all solicitation requirements.
