About this file
This document is a Foreign Country of Concern Attestation form (PUR 1355) issued by the Florida Department of Management Services for the Management Consulting Services RFP No. 25-80101500-RFP-V2. The form is a mandatory certification that must be completed by an officer or representative of an entity submitting a bid, proposal, or contract with a governmental entity that would grant access to personal identifying information. The attestation requires the bidding entity to confirm that it is not owned by, organized under the laws of, or controlled by the government of a Foreign Country of Concern.
The form is a standard state procurement requirement designed to ensure that contractors do not have significant foreign government connections that could potentially compromise sensitive information. By signing under penalties of perjury, the authorized representative declares the truthfulness of the statement, providing a legally binding assurance about the entity's independence from foreign government control. This attestation is part of a broader set of mandatory certifications required for participation in the Florida Department of Management Services' procurement process, specifically for the Management Consulting Services contract opportunity.
