About this file
Attachment I is a Foreign Country of Concern Attestation form (PUR 1355) issued by the Florida Department of Management Services for the Management Consulting Services RFP No. 25-80101500-RFP-V2. The form is a mandatory certification document that must be completed by an officer or representative of an entity submitting a bid, proposal, or entering into a contract that would grant access to Personal Identifying Information. The form requires the bidding entity to affirm that it is not owned by, organized under the laws of, or controlled by the government of a Foreign Country of Concern.
The attestation is part of a broader procurement process for a new State Term Contract for Management Consulting Services, with the state reserving the right to make multiple statewide awards across nine service categories. The previous contract had an annual spend of approximately $90 million in Fiscal Year 2023-2024, though future spending is not guaranteed. Vendors must be registered in the MyFloridaMarketPlace Vendor Information Portal, complete various mandatory certifications, and comply with state requirements such as using E-Verify for new employees and maintaining a drug-free workplace. The procurement involves detailed technical proposals evaluated on a 100-point system, with 90 points for the Technical Proposal and 10 points for the Cost Proposal, covering service categories like Organizational Strategy Development, Project Management Consulting, and Comprehensive Grants Management.
