About this file
This is a COBID Outreach and Certification Form attachment issued by the State of Oregon Department of Environmental Quality (DEQ) in conjunction with DAS Procurement Services for Solicitation #S-34000-00016785 regarding the design, fabrication, and transportation of an Emission Capture Hood for Pile Burn Testing. The form requires offerors to disclose their certification status under Oregon's Certification Office for Business Inclusion and Diversity (COBID) program, which certifies minority-owned businesses, woman-owned businesses, service-disabled veteran-owned businesses, and emerging small businesses. Offerors must document any prior contractual relationships with certified firms within the past two years and identify potentially eligible uncertified businesses with which they have worked.
For offerors anticipating subcontracting opportunities, the form requires submission of a comprehensive outreach plan detailing steps to solicit certified firms, descriptions of specific subcontracting opportunities with approximate dollar values, and willingness to report subcontractor identities and values to COBID upon contract award. The certification and outreach information submitted will not be considered in the scored evaluation of proposals. Completion and submission of all form pages is mandatory only if subcontracting opportunities are anticipated; otherwise, only the first page need be submitted with the offer.
