About this file
This is an Attachment D Administrative Code Chapter 12L Compliance form for a Request for Proposals issued by the San Francisco Department of Public Health's Behavioral Health Services (BHS) division seeking regulatory and policy consulting services. The RFP (SFGOV-0000011831) requests proposals from experienced firms to provide professional staff for regulatory consulting, policy analysis, program development, project management, data analysis, training, and interagency coordination to support behavioral health policy implementation and system operations. Key solicitation milestones include a pre-proposal conference on August 24, 2026 at 10:00 AM PT via Microsoft Teams, a deadline for questions on August 31, 2026 at 5:00 PM PT, an estimated Q&A addendum posting on September 8, 2026, a proposal submission deadline of September 22, 2026 at 2:00 PM PT, an estimated award announcement on October 30, 2026, and an estimated contract start date of July 1, 2027.
The compliance form specifically addresses Chapter 12L requirements applicable to non-profit entities that receive a cumulative total of at least $1,000,000 per year in City funds or City-administered funds. Proposers meeting this threshold must complete two compliance items: Item #1 requires a statement describing efforts to comply with Chapter 12L provisions regarding public access to meetings and records, and Item #2 requires a summary and disposition of all substantiated complaints concerning Chapter 12L compliance filed with the City within the last two years. Non-profits receiving less than $1,000,000 annually in City funds and for-profit entities are exempt from this compliance requirement. Failure to comply with Chapter 12L reporting requirements or material misrepresentation in submissions constitutes grounds for proposal rejection and/or contract termination.
