About this file

This document is a vendor experience documentation form for a Statewide Natural Areas Management contract administered by the Illinois Department of Natural Resources. The form establishes requirements for vendors to demonstrate qualifications across multiple work categories and personnel roles.

The contract encompasses seven distinct work items: handwork in sensitive sites, handwork in non-sensitive sites, grinding/forestry mowing in sensitive sites, grinding/forestry mowing in non-sensitive sites, grinding/forestry mowing with an amphibious vehicle, firebreak maintenance, and foliar/wicking herbicide application from an amphibious vehicle. Vendors must submit separate experience documentation for each work item, with reference projects demonstrating required experience duration and site type appropriateness. Experience requirements vary by work item and zone as specified in Section 1.4.3 of the contract specifications. Key personnel documentation is required for crew leaders (all work items except grinding/forestry mowing), machine operators (grinding/forestry mowing), and laborers (all work items except grinding/forestry mowing with an amphibious vehicle). The form requires vendors to provide project site information, reference contacts, land use type, acreage, work type, and completion dates. Personnel substitutions are permitted if replacements meet applicable experience requirements, with the Illinois Department of Natural Resources retaining the right to request documentation of replacement personnel qualifications. Crew leaders must maintain on-site presence during all work operations.
