About this file
The Bidder Status Form is an official document approved by the Iowa Labor Commissioner, designed for state procurement processes related to the Thomas Mitchell Stream Restoration Project in Eastern Polk County, Iowa. The form helps determine whether a bidding company qualifies as a resident or non-resident bidder by evaluating the company's business status, authorization to transact business in Iowa, and length of operational history within the state. Companies must complete different sections (Parts A-D) depending on their residency status, with resident bidders needing to demonstrate at least three years of continuous business operations in Iowa and non-resident bidders required to disclose their home state and any preferences offered there.
The form requires detailed documentation about a company's business authorization, including registration with the Iowa Division of Labor, partnership compositions, corporate standing with the Iowa Secretary of State, and specific organizational structures like limited liability companies or partnerships. Bidders must certify the accuracy of their submitted information, understanding that providing false or incomplete details could result in bid rejection. The worksheet accompanying the form provides comprehensive guidance on what constitutes authorization to transact business in Iowa, offering multiple pathways for businesses to qualify as authorized entities for bidding purposes.
