About this file

This document comprises two attachments—Responder Declarations and Exceptions to State's Terms and Conditions—associated with a Minnesota Department of Information Technology Services (MNIT) and Department of Corrections (DOC) solicitation for Survey and Assessments Solutions. The solicitation seeks a secure, scalable solution to support the complete lifecycle of surveys and assessments administered to incarcerated persons within Minnesota Correctional Facilities, including design, distribution, management, response collection, and automated scoring according to DOC-defined algorithms. The solution must provide flexible reporting capabilities for DOC staff to view and download scored results, integrate with the DOC's Operations Management System to securely ingest structured data elements related to incarcerated persons' status and history, evaluate that data through DOC's internally developed scoring algorithms, and generate supplemental analytical outputs and reports. The solution must incorporate role-based access control to protect sensitive assessment data and ensure accessibility only to authorized personnel. The document does not specify response due dates, site visits, bidder conferences, award dates, or contract term details.

Attachment A requires responders to certify the accuracy and reliability of submission information, confirm authorized signature authority, attest to non-collusion, disclose any organizational conflicts of interest, provide lobbying certifications for contracts exceeding $100,000, and confirm copyright clearances. Attachment B provides a template for responders to identify and document any exceptions to the State's terms and conditions found within the solicitation or sample contract, requiring explicit listing of each exception with reference to clause and page numbers, or declaration of "NONE" if no exceptions exist. Both forms must be signed and dated by an authorized representative as part of the response submission.

D. Organizational Conflicts of Interest. To the best of Responder’s knowledge and belief, and except as otherwise disclosed, there are no relevant facts or circumstances which could give rise to an organizational conflict of interest.

If an organizational conflict of interest is determined to exist, the State may, at its discretion, cancel the contract. In the event the Contractor was aware of an organizational conflict of interest prior to the award of the contract and did not disclose the conflict to OSP, the State may terminate the contract for default. Organizational conflicts of interest terms apply to any subcontractors for this work.
