About this file

This is a Replacement Case Protocol document prepared by the Missouri Department of Social Services for the Foster Care Case Management contract opportunity. The protocol establishes the procedures and requirements for managing case replacements when a caseworker transitions out of a foster care case. The document outlines the systematic process for identifying cases requiring reassignment, documenting the reason for replacement, and ensuring continuity of care for children and families in the foster care system. The protocol specifies timelines for case transfer activities, including notification requirements, documentation procedures, and the responsibilities of both the departing and incoming caseworkers to ensure minimal disruption to foster care placements and services.

The replacement case protocol emphasizes maintaining comprehensive case records and communication between caseworkers during transitions. The document details the steps for reviewing case files, scheduling meetings between the outgoing and incoming case managers, and establishing clear handoff procedures to preserve institutional knowledge and case history. Additionally, the protocol addresses documentation requirements for case reassignments and establishes accountability measures to ensure that all foster care cases maintain appropriate oversight and service continuity throughout the transition period.
