ADDENDUM TO FAR 52.212-1, INSTRUCTION TO OFFERORS—COMMERCIAL ITEMS

THIS SECTION IS FOR SOLICITATION PURPOSES ONLY. THIS SECTION WILL BE PHYSICALLY REMOVED FROM ANY RESULTANT AWARD, BUT WILL BE DEEMED INCORPORATED BY REFERENCE.

Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.

ADDENDA TO 52.212-1- PROPOSAL PREPARATION INSTRUCTIONS

NOTE: All proposal preparation cost will be the sole responsibility of the offeror. The Government will not reimburse any firm for their proposal preparation cost.

1. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications. Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of two (2) separately bound parts, Part I – Price Proposal and Part II – Past Performance Information.

2. General Information

2.1. INFORMATION REGARDING SUBMISSION OF PROPOSALS: Hand carried proposals must be delivered to 7505 Barnes Loop, Bldg. 208, F. E. Warren AFB, WY 82005. (E-mailed proposals are preferred but not required). The sealed envelope or package used to submit your proposal must show the time and date for receipt, the Solicitation Number, and the name and address of the offeror. Mailed proposals (USPS, FedEx, UPS, DHL, etc.) must be sent to the following address:

2.2. If the offerors e-mail attachment size exceed 20MB, they will exceed current Air Force e-mail attachment size and the e-mail may not be received. If the offeror has files that exceed 20MB, please contact the Contract Administrator, SrA Jamie Meier at jamie.meier.1@us.af.mil for instructions on how to submit files through DoD SAFE, https://safe.apps.mil/.

2.3. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:

2.3.1. Requirements of the RFP (i.e., SF1449, Attachments and government standards and regulations pertaining to the PWS.)

2.3.2. Evaluation Factors for Award.

2.3.3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in 52.212-1 will not be read or evaluated, and will be removed from the proposal.

2.4. Format for proposal parts shall be as follows:

2.4.1 A page is defined as one face of an 8 ½” x 11” sheet of paper containing information.

2.4.2. Typing shall not be less than 11 pitch.

2.5. Offerors are cautioned that F. E. Warren AFB has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the 90 CONS to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.212-1(f) ―Late submission, modifications, revisions, and withdrawals of offers.

2.6. Site Visit Information: A Pre-Proposal Site Visit will be held on 30 Apr 2020 at 09:00am MST at F.E. Warren Air Force Base. All interested parties must submit names of attendees to jamie.meier.1@us.af.mil by 02:00pm MST on Monday, 27 Apr 2020. A Pre-Proposal Briefing will be held at 90 CONS Conference Room prior to the Site Visit. All attendees who do not have access to F. E. Warren Air Force Base, please meet at the Gate 1 off Interstate 25 at 08:30am MST for base pass and escort. ALL QUESTIONS POST SITE VISIT MUST BE SUBMITTED TO jamie.meier.1@us.af.mil NO LATER THAN 1May 2020 by 04:00pm MST.

2.7. FEDERAL HOLIDAYS: The following Federal Legal Holidays are observed by this base:

New Year’s Day – 1 January Martin Luther King’s Birthday – Third Monday in January Presidents Day – Third Monday in February Memorial Day – Last Monday in May Independence Day – 4 July Labor Day – First Monday in September Columbus Day – Second Monday in October Veterans Day – 11 November Thanksgiving Day – Fourth Thursday in November Christmas Day – 25 December

NO CORRECTIONS AND/OR CHANGES ARE ALLOWED AFTER TIME OF SUBMISSION OF PROPOSAL OR BIDS.

3. Specific Instructions

4. PRICE PROPOSAL

Format for price proposal shall be as follows;

4.1. Complete the SF 1449, blocks 12, 17a, 30 (a), (b) and (c). An official having the authority to bind your company contractually must sign and date the SF 1449 Block 30b. Submit one (1) of this part. The SF 1449 must bear an original, authorized signature. In doing so, the Offeror accedes to the contract terms and conditions as written in the Request For Proposal (RFP). All sections of the RFP, excluding Instructions to Offerors and Evaluation Procedures Sections, constitute the model contract.

4.2. Complete all CLINs in the SF 1449 Continuation Sheet Schedule, to include all option periods. To comply with an AF template for this service, price the CLINs. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal must be submitted for a base period plus four (4) option periods. Unit prices shall prevail in the event of a mathematical error when calculating the extended price.

4.3. Complete the necessary fill-ins and certifications in provisions. The provision FAR 52.212-3 shall be returned along with the proposal.

4.4. Acknowledge any and all amendments to the solicitation.

4.5. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional information will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.

4.6. The price proposal will be reviewed for responsiveness, an incomplete proposal may result in removal from competition. Unrealistically low price or cost estimates, initially or subsequently, may be grounds for eliminating a proposal from competition. The Government may use various price analysis techniques and procedures to ensure a fair and reasonable price. Examples of such techniques include, but are not limited to the following: Price analysis: The process of examining and evaluating a proposed price without evaluating its separate cost elements and proposed profit. Comparison of proposed prices received in response to the solicitation. Normally, adequate price competition establishes price reasonableness. Comparison of previously proposed prices and previous Government contract prices with current proposed prices for the same or similar effort. Additionally, the proposal will be reviewed for unbalanced pricing. An offer may be rejected if the contracting officer determines prices are significantly unbalanced or if the unbalanced pricing poses an unacceptable risk to the Government.

5. PAST PERFORMANCE

Format for past performance proposal shall be as follows;

5.1. Limited to no more than 2 pages per contract listed. Only references for the same or similar type contract desired. Submit one copy.

5.2. Quality and Satisfaction Rating for contracts completed in the past three (3) years: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for same or similar type contract. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.

5.2. Past Performance Survey (Attachment 8). The Government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The Government will use information submitted by the offeror and other sources such as other Federal Government offices and commercial sources, to assess performance. Provide a list of no more than five (5) of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. Past Performance includes contract efforts that are currently being performed. Relevant contracts include work of a nature, magnitude and scope similar to that required for this contract within the past three (3) years from date of solicitation. 5.4. Past performance regarding predecessor companies, key personnel who have relevant experience, or sub-contractors that will perform major or critical aspects of the requirement may be included as relevant past performance. Furnish the following information for each contract listed:

5.4.1. Contracting Agency/Customer

5.4.2. Contract Number

5.4.3. Contract type

5.4.4. Project title and brief description of contract effort

5.4.5. Period of performance

5.4.6. Contract value at time of award

5.4.7. Current value, including all changes and exercised option

5.4.8. Contracting Officer’s name, address and telephone number

5.5. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts.

5.6. Each offeror is responsible for sending the attached past performance survey to its chosen references. Upon completion of the survey, they shall be returned directly from the references to the attention of 90 CONS/PKAB, SrA Jamie Meier, 7505 Barnes Loop, Bldg. 208, F.E. Warren AFB, WY 82005. This list may be provided (preferred) by email to jamie.meier.1@us.af.mil. Completed survey must be submitted no later than 1 May 2020 1:00pm MST. This requirement for the submission of survey is in addition to the requirement for a list of consecutive contracts.

5.7. Offerors are cautioned that the Government will use information provided by the offerors and information obtained by other sources in the development of the past performance rating.

5.8. Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
