PART 1 GENERAL

1.1 SUBMITTALS

SUMMARY OF WORK

Government approval is required for all submittals. This includes submittals with or without a "G" designation. Submit items in accordance with the AF3000 Material Approval Submittal Procedures. For example:

“01 12 36 41 00 Mechanical - Water Chiller, Centrifugal Type, Trane, Johnson Controls/York - Model Number: XYZ000XYZ000”

1.2 WORK COVERED BY CONTRACT DOCUMENTS

1.2.1 Project Description

The work includes Demo and Replacement of: the Centrifugal chiller, DDC EMCS, pumps, and of limited piping, hangars, electrical circuits and components, appurtenances and incidental related work. The Contractor shall review these specifications. Some specifications have open parentheses and/or blanks. These are designed for the Contractor to review, determine applicability and tailor as needed to their proposed construction method then submit for review as part of the Contractor’s overall “design analysis”. There is no required “formal design” meaning the Government does not believe engineered design is required for any portion of this contract. The term Design then, is to refer to manufacturer’s design and drawings, CAD technician level shop drawings individual trades shop drawings, and a final set of as-built drawings in .dwg and .pdf formats.

1.2.2 Location

The work is located at ‘B’ Plant, building 10056, Joint base San Antonio Lackland 78236. Refer to the appendices for Facility Data. The exact location will be shown by the Contracting Officer.

1.3 OCCUPANCY OF PREMISES

This project is within the confines of a Chill Water Plant; an unoccupied facility, however, the buildings that it serves shall be occupied during performance of work under this Contract.

Before work is started, arrange with the Contracting Officer and the CEPM a clearly defined and itemized sequence of procedure(s), for any required portable chiller and plant shutdowns, space for storage of materials and equipment, and use of approaches.

1.4 EXISTING WORK-DEMOLITION

In addition to "FAR 52.236-9, Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements":

a. Remove or alter existing work in such a manner as to prevent injury or damage to any portions of the existing work which remain.

b. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work must be in a condition equal to or better than that which existed before new work started.

c. The Contractor shall Remove the existing 1075 Ton water-cooled chiller, and chilled water supply and return piping system in the mechanical room (existing pipe system can be reused as practicable).

Existing condenser and chilled water piping from the Chiller plant to the Condenser shall remain. Contractor shall recover all refrigerant 123 from the chiller and deliver all refrigerant to the JBSA Refrigerant Shop, B. 485 Chapman Training Annex. The Contractor shall obtain recovery vessels from the refrigerant shop prior to start of demolition by contacting the CEPM or CEI a minimum 5 business days prior to the Licensed Contractor’s intended removal date. The Contractor shall provide copies of all licenses as part of their QC Plan and shall ensure licensees bear those licenses on their person during the execution of this project. It is anticipated that the existing portable chill water connection points will require change/modification to allow for equipment removal. The ability to keep the temporary portable chiller functioning as support to the adjacent chiller is mandatory, therefore the Contractor shall demo the existing extraneous exterior hook-up points and prepare a proper exterior connection point prior to any other demolition work. Once the new points are established and the portable chiller can be moved and reconnected by the Government, The Contactor shall continue with required work elements to include the removal of one primary loop pump and two secondary loop pumps and prepare for new. Housekeeping pads shall not be removed unless they have failed, or are no longer suitable for the selected new pump's nomenclature.

d. The Contractor shall remove all Chilled water valves and actuators in the mechanical room. The Contractor shall demo those sensors and thermostats necessary and prepare for new.

e. The Contractor shall remove the existing DDC control systems and prep for new. Contractor shall NOT simply cut wires and remove. The Contractor shall carefully, clearly, and purposefully identify and mark wiring inside the Direct Digital Control Panels as necessary to ensure all circuits and devices intended to be saved, or pass-through circuits and signals are preserved for reinstallation. Communications (CAT) and any fiber optic will be clearly marked and identified as to where they run. Communications to the base DDC network shall not be removed, however all panels currently housing the connection to the media converter and base network shall be replaced with a new combined panel. The Contractor shall be responsible for maintaining the connections and continuity to the campus wide network. The existing CAT connection point (jack) located on the wall between the EMCS panels shall be removed and replaced with new, however, the Contractor shall provide and install a new panel to house this connection.

f. The Contractor shall demo and remove any electrical conductors, switches, overcurrent devices and panels necessary to prepare for new.

Nothing shall be abandoned in place. All conductors and circuits intended to be removed shall be removed completely back to its Over Current Protection Device. Any conduit, electrical and communications box, panel etc., that is currently not used shall be demolished and removed.

g. The Contractor shall demo and prepare for new, the existing make up water system.

1.4.1 NEW WORK

a. The Contractor shall provide, and install a new replacement water cooled centrifugal chiller of same size and all its appurtenances compatible with the specification shown in the chiller schedule below. The Chiller may be Trane or Johnson Controls-York. The chiller shall be installed such that it shall meet the Manufacturer's requirements, captures the Manufacturer’s Warranty and meets the Manufacturer's Start-up Procedures that shall be conducted by the Manufacturer. The chiller and its piping shall meet the existing design for water flow, and heat load.

b. The Contractor shall install new/modify existing chilled water and condenser water piping in the vicinity of the chiller as necessary to connect the new chiller to existing piping. Contractor shall install new thermal insulation on new/modified chilled water piping.

Contractor shall connect existing chiller electrical service to new chiller. New chiller shall incorporate a single terminal block. All chiller piping, power and control wiring shall be run neatly, at right angles and properly affixed in such a manner as to allow for easy servicing of the units. All chilled water piping shall be insulated with a minimum 2” all-weather jacketed cellular fiberglass insulation. All seams shall be completely coated with a minimum exterior grade mastic such that no gaps, voids or air bubbles are present.

c. DDC: All HVAC components in building 10056 are currently connected to a Direct Digital Control (DDC) system and existing installation-wide Tridium® based EMCS front end that currently uses Niagara based BACNET Protocol. The EMCS front end is currently operated and maintained in building 5595, room 111, JBSA-Lackland. Contractor shall provide and install all graphics and averaging graphing to the supervisory level to match the Front End. Contractor shall provide all conduits, cables, termination boxes, and any additional items/accessories necessary to connect each of the HVAC components installed as part this contract back to the existing DDC controller.

All new HVAC components (points) shall be mapped back and relativized to the front end. Sample of the Tridium® front end graphics can be provided to the contractor upon request in PowerPoint format. This system shall be proven to operate, by the contractor, at the front end via a Point Verification Test (PVT).

The PVT shall be performed a minimum of 7 days prior to the TAB (or longer should the TAB Agent so desire) to allow for corrections to be made to the system before commencement of the 3rd party tests.

d. Throughout this project all new controllers, valves, sensors and points shall be brought to the existing facility wide JCI NS Series system and to the METASYS®/Tridium® front end located at B5595 Room

111. All new points shall be proven to be read and be manipulated/toggled (work) at the front end. A Point Verification Test (PVT) shall be satisfactorily completed a minimum of 5 duty days prior to the TAB to allow for corrections to be made to the system before commencement of the 3rd party tests. The PVT agent shall coordinate with the CEPM, and CEI a minimum 5 business days in advance to allow for the coordination with the Front-End, the Government’s DDC Contractor and other Government DDC specialists.

e. Contractor shall protect and preserve the existing com fiber connection

SECTION 01 11 00 Page 4 within the facility for re-use. Should damage to this item, its functionality or to its appurtenances occur, the Contractor shall provide new communications conduit, fiber optic cable, termination box, and any additional items/accessories from the necessary to reconnect to the base network. The LAN/data outlet shall be single outlet type. The single outlet shall contain one (1) LAN/data port. The contractor shall coordinate effort with the 502 CES PM to tie in the new comm.

f. All valves shall be numbered and identified with a round, solid brass tag. A complete diagram and listing of the valves shall be provided with the O&M package.

g. Contractor shall clean and flush the condenser water loop prior to operation of new chiller. Contractor shall provide factory start-up.

Contractor shall provide a five year parts, labor and refrigerant warranty for the chiller.

h. The contractor shall install a new Taco 2524 horizontal split case condenser water pump and motor assembly compatible with the condenser water pump schedule below and in the same location as the existing Taco horizontal split case condenser water pump. Contractor shall install new condenser water piping as necessary to connect existing condenser water piping to new condenser water pump. Contractor shall connect existing condenser water pump electrical service to new condenser water pump. Replacement pump assembly shall include shaft grounding.

i. The contractor shall overhaul one Taco split case TA 1224 20 HP primary loop chilled water pump, to include centrifuge, seals and gaskets to manufacturer’s specifications. Contractor shall replace pump motor and provide shaft grounding for this pump.

j. The contractor shall overhaul one Taco split case TA 2524 50 HP primary loop chilled water pump to include centrifuge, seals and pump motor and provide shaft grounding for this pump.

k. Contractor shall overhaul two Taco split case 2530 75 HP secondary loop pump chilled water pumps to include centrifuge, seals and pump motors and provide shaft grounding for these pumps.

l. Contractor shall replace the motor shaft grounding for one Taco

1524 30 HP condenser water pump.

m. All electrical motors installed under this contract shall be compatible with existing electrical service. All Pumps and motors shall be documented laser aligned. A copy of the documentation shall be submitted to the Government for review. The Contractor shall provide a copy of the approved documentation with the O&M Binder(s).

n. The Contractor shall provide and install all piping, connectors, fittings, mounts, hangars, and all necessary appurtenances to prove a fully operating Chiller and supply and return water loop capable of meeting the design demand.

o. The Contractor shall provide and install all properly sized and

NFPA 70 compliant conduit, conductors, overcurrent devices, panels, drives, and appurtenances necessary to prove a fully functioning Chiller, controls and water circuit pump system.

p. The Contractor shall provide the material and workmanship necessary to produce a quality coating system. All painting work shall be implemented in strict accordance with the manufacturer's instructions and shall be performed in a manner satisfactory to the CEPM. Contractor shall prepare and repaint the existing expansion tank to Chiller 2. The Contractor shall drain, remove, purge and prepare the tank for painting using thus the Contractor shall expect to include necessary standard practices to accomplish this. Remove all oil, grease, and existing loose paint from the surface prior to abrasive cleaning. All exterior surfaces shall be abrasive cleaned to a Commercial Finish, removing all existing paint, rust, dirt, mill scale and foreign matter by the recommended methods outlined in the SSPC Society of Protective Coating's Specification SP-6.

Immediately after abrasive cleaning and before any rusting occurs (within 12 hours maximum), apply one coat of approved high build grey primer to all bare steel surfaces. This coating shall be manufacturer approved for use as a substrate for the 2 part epoxy topcoat and applied at a dry film thickness of 2.5 - 3.5 mils. Apply to manufacture’s specification, a High Build, medium to low VOC, A two-component, high-solids polyamidoamine, cycloaliphtical epoxy at a final dry film thickness of 3.0 mils. Color of finish shall be an approved, industry standard Mill White.

q. All paint shall be applied in strict accordance with the applicable manufacturer's printed data sheet and container label outlining recommended minimum and maximum surface and air temperatures required for application. No paint shall be applied when it is expected that the relative humidity will exceed 75% and/or the air temperature will drop below recommended levels within 12 hours after paint application.

Painting shall be delayed until certain that the surfaces are dry.

r. Dry film thickness shall be measured in accordance with current SSPC

PA2, "Dry Paint Thickness with Magnetic Gauges".

s. The Contractor shall demo all insulation on any pipe and device/equipment as necessary to conduct the definable features of work within this project. The contractor shall seal all new pipe with a "no condensate" low VoC sealer. Once the application of this sealer is inspected as satisfactory by the CE Inspector and Project Manager, the Contractor shall reinsulate with an industry standard pre-formed fiberglass pipe insulation with an ASJ jacket. For the purpose of this installation, the Contractor shall seal, staple and over-seal all joints and repairs with a minimum 2 coats of exterior grade low VOC mastic. All joints shall be without gaps, voids and other weak spots.

t. The Contractor shall properly size a new make-up water line from the facility’s existing water service line located in the SW corner of the 1000 ton chiller bay. The Contractor shall provide and install a new, properly sized Backflow Device and ensure water quality at the entry point to the distribution system is protected. The backflow device shall be compliant with the Manual for Cross-Connection Control, Guide to Cross-Connection Protection Devices and Assemblies – Third Edition (Current publication date is that at time of solicitation advertisement). The BFD shall be installed, and tested by a currently licensed Texas Backflow Assembly Tester, and the test shall be completed IAW Title 30 of the Texas Administrative Code 290.44(h)(4)(c). This test shall be conducted using the TCEQ Backflow Assembly Test Form located in these specifications’ appendices. The Contractor shall notify the CO, CEPM and CEI no less than 7 days prior to schedule the test so that they or their representative can observe, validate the Tester’s credentials, and sign. Either CEPM or the CEI shall sign in the “owner block”. An Original signed copy shall be provided to the CEPM within 72 hours of the test. The existing

Backflow device shall be turned over to the Government via the CEPM.

u. TESTS and INSPECTIONS: All tests and inspections throughout this entire project from start to Warranty Complete shall be done with a qualified discipline specific representative of CE, CE Inspector and the Contractor present. A test without these three present is no test and not acceptable. It is the Contractor’s responsibility to plan ahead.

This is a small project and as such, there shall not be a “pre-final Inspection.” The Contractor shall be solely responsible for their own Quality Control Program and the on-site management of that program. As such, the Final Inspection shall be go or no-go. Any Major Findings and or shortcomings will result in a no-go. The following are examples and are some, but by no means all possible Major Findings: A DDC Point does not operate, or the chiller does not hold the load for 24 hours, or a pump and/or its motor does not function properly. No more than 10 Minor Findings and or shortcomings will result in a no-go. The following are examples and are some, but by no means all possible Minor Findings: Gaps in mastic, poor workmanship such as sloppy wiring, lack of labels, untagged valves and unlabeled overcurrent protection devices inside any panels. Throughout this project the CO, CS, CEPM, CEI, 502 ABW Safety, or LAFD will visit the site announced or unannounced.

Observations and findings made during said visits are for the sole purpose of the Government and will not be shared with the Contractor.

1.5 LOCATION OF UNDERGROUND UTILITIES

Obtain the current AF103 Base Civil Engineering Work Clearance Request from the 802 CEPM or 802 CEI prior to the start of any excavation, or ground penetrating preparation and comply with Installation requirements for locating and marking underground utilities. Contractor shall Contact TX 811 and American Water’s locating service a minimum of 72 prior to excavating, to mark utilities, and within sufficient time required if work occurs on a Monday or after a Holiday.] The Contractor is responsible to verify existing utility locations as marked and maintain markings until excavation or operation is complete.

Should something is found to be suspect and not identified by marking(s), the Contractor shall identify and mark all other utilities not managed and located by the local utility companies. This may or may not entail scanning the construction site with Ground Penetrating Radar (GPR), electromagnetic, or sonic equipment, and mark the surface of the ground or paved surface where existing underground utilities are discovered/suspected. Verify the elevations of existing piping, utilities, and any type of underground or encased obstruction not indicated, or specified to be removed, that is indicated or discovered during scanning, in locations to be traversed by piping, ducts, and other work to be conducted or installed.

1.5.1 Notification Prior to Excavation

The Contractor shall notify the Contracting Officer at least 72 hours prior to starting excavation work.

1.7 SALVAGE MATERIAL AND EQUIPMENT

The Contractor shall turn over the existing Backflow Prevention Device to the Government via receipt to the CE Project Manager.

PART 2 PRODUCTS

PART 3 EXECUTION

Due to the multi-facility, multi-mission effect of this facility should it complete go down, the Contractor shall provide a carefully laid out EXECUTION PLAN for this project. This plan shall outline the approach to demolition and timeline, the approach to repair and replacement and timeline. This plan is in addition to, but shall accompany, the Contractor’s required Project Schedule. This plan shall provide a solid overview of the intended business process of the Contractor so that the Government can provide top cover to the contractor and know to whom it must coordinate with. This plan is to be submitted using the AF3000 process a maximum 10 days after the Notice to Proceed and before any Demolition and Construction begins.

1.1 REFERENCES

The publications listed below form a part of this specification to the extent referenced. The publications are referred to within the text by the basic designation only. No radioactive materials are a part of this Scope, however, testing equipment used by some contractors does contain radioactive components and as such, transportation, handling, storage and use shall comply with the document.

1.1 SUBMITTALS

Government approval is required for submittals with a "G" designation;

submittals not having a "G" designation are for Contractor QC approval and submission to the Government as Information only. Submit the following in accordance with this section.

1.2 SPECIAL SCHEDULING REQUIREMENTS

All mission critical areas will remain in operation during the entire construction period. The contractor shall conduct his operations so as to cause the least possible interference with normal operations of the activity. This is to include ingress/egress of the jobsite. During certain times of the day assembly and movement of troops will be present in adjacent parking lots and sidewalks. Though the leadership of these units will be aware of your presence, total elimination of their assembly and movement is not feasible.

This project site is located in the Navy and Air Force training dormitory area of JBSA Lackland. Permission to interrupt any Activity roads, mission partner missions, and/or utility service shall be requested in writing a minimum of 30 calendar days prior to the desired date of interruption. The Contractor shall maintain close and active communications with the JBSA Project Manager and Contracting Officer such that any outage or interruption is planned for as far in advance as possible.

1.3 CONTRACTOR ACCESS AND USE OF PREMISES

1.3.1 Activity Regulations

Ensure that Contractor personnel employed on the Activity become familiar with and obey Activity regulations including safety, fire, traffic and security regulations. Keep within the limits of the work and avenues of ingress and egress. Wear Personal Protective Equipment In Accordance with (IAW) OSHA Law and EM 385-1 in designated areas. Do not enter Restricted, Controlled or limited access areas unless required to do so and pre-cleared and pre-announced for such entry Failure to do so may result in detention, apprehension, arrest or any combination thereof. The Use of Deadly Force is authorized on JBSA Installation. Mark all Contractor equipment for identification. The Contractor is responsible for the security of their materials and equipment.

1.3.1.1 Subcontractors and Personnel Contacts

The Contractor shall provide a complete list of contract personnel of the Contractor and subcontractors including addresses and telephone numbers, for use in the event of an emergency, to the Contracting Officer (CO) and Civil Engineer Project Manager (CEPM). As changes occur and additional information becomes available, The Contractor shall correct and change the information contained in previous lists, before the Government has to ask.

1.3.1.2 Electronic Mail (EMAIL) Address(es)

The Contractor shall establish and maintain electronic mail (e-mail) capability along with the capability to open various electronic attachments in Microsoft, Adobe Acrobat, and other similar formats. Within 10 days after contract award, the Contractor shall provide the Contracting Officer a single (only one) e-mail address for electronic communications from the Contracting Officer related to this contract including, but not limited to contract documents, invoice information, request for proposals, and other correspondence.

The Contractor shall endeavor to protect DoD Data. The Contracting Officer may also use email to notify the Contractor of base access conditions when emergency conditions warrant, such as hurricanes, terrorist threats, etc.

Multiple email address will not be allowed.

All DoD and Government data that is shared, stored or created on non-DoD information systems must be protected and any breach or loss of that data must be reported immediately, not to exceed 24 hours. The contractor shall comply with the security requirements defined in DoD Instruction 8582.01.

The use of private email addresses (Yahoo, Gmail etc.) by the Contractor, sub-contractors or other entities involved in this project, for project related information, are not acceptable. At this level of business, the Contractor is expected to provide a company or corporate .com, .biz or other ICANN-accredited and registered domain with appropriate security protocols in place.

It is the Contractor's responsibility to make timely distribution of all Contracting Officer initiated e-mail with its own organization including field office(s). The Contractor shall promptly notify the Contracting Officer, in writing, of any changes to this email address.

1.3.1.3 Installation Access

Non DOD persons (Contractors, vendors, etc.) requiring entry to JBSA installations shall be sponsored by an authorized DBIDS registered DOD ID card holder.

The Contractor, in compliance with the Federal Real ID Act, shall provide a complete and typed columnar list of Contractor and sub-contractor employees that will be assigned on this Project. This list shall contain for each name listed: Last Name, First Name, Middle Initial, Date of Birth, Country of Birth, Type of Identification Document (e.g. driver's license, Permanent Residence Card), Identification Document Number, Country of Issue, State of Issue and Citizenship. This list shall have a Contractor statement that all personnel's citizenship has been physically validated by their employer.

Should a Contractor chose to employ Non US or Non-naturalized workers on this project, the vetting process for these personnel is a minimum 90 day process, as additional time is required for the international governmental cooperation to occur.

The following is also required for passes 60 days or longer:

a. A memo from the construction company on company letterhead stating the reason for entry, contract number, project start and end date, and the location at JBSA of where the jobsite is located.

b. The Contractor shall identify all key and signatory personnel with an asterix next to their name and include a letter on company letterhead, signed by a principle, stating their signatory abilities to the Contracting Officer.

1.3.1.4 JBSA Lackland Access

Person(s) not in possession of a valid access credential must enter the installation through the Luke East Visitor’s Control Center in order to have their Installation Access pass issued to them.

Commercial vehicles such as delivery vehicles, shop trucks, public utility, etc., with Installation Access passes shall enter the installation through the Growdin Rd. Commercial Gate.

1.4.1.3.1 Haul Routes

Travel only on defined roads through the JBSA-Lackland Haul Route identified in the Appendices of this Scope.

1.4.1.3.2 Gate Restrictions

All commercial Class A/B type vehicles must enter through:

a. The Commercial Gate at Growden Road.

c. Arrangements may be made on rare instances to waive access protocol to allow Class A/B type vehicles through other gates for special circumstances on a case by case basis. Each case needs a minimum 72 business hour notification to the CO and CEPM. The Contractor shall provide the Contractor or sub-contractor name, number of persons and vehicles, make model, type of vehicles, and their license plate data.

The Contractor shall also be provided in addition to their Installation Access request information as noted in paragraph 1.4.1.2 above.

1.4.1.4 Government Housing Areas

a. Contractors, vehicles and their subcontractors shall NOT go through the housing, commercial commerce or residential areas of JBSA at any time.

1.4.1.5 No Smoking Policy

Smoking is prohibited within and outside of all buildings on JBSA installations, except in designated smoking areas. This applies to existing buildings, buildings under construction and buildings under renovation.

Discarding tobacco materials other than into designated tobacco receptacles is considered littering and is subject to fines.

1.4.2 Emergency Response Requirement

Contractors reporting an emergency shall provide the 9-1-1 dispatcher with the following information:

a. Installation name and Building number, address or general location

b. Full Name

c. Phone number

d. Emergency information (cause of fire, explosion, natural gas leak, injuries, etc.)

e. Please, do not hang up the phone until directed by the 9-1-1 dispatcher.

IMPORTANT: If calling 9-1-1 from a mobile phone on any JBSA location you will be directed to the San Antonio Emergency Operations Center 9-1-1 dispatcher. Advise the 9-1-1 dispatcher of your JBSA location (Information in Paragraph 1.4.2 a. above. Immediately, the local dispatcher will transfer the 9-1-1 phone call to a JBSA emergency dispatcher.

1.4.3 [Enter Appropriate Subpart Title Here]Radioactive Materials and Equipment (Use and Storage) IMPORTANT: Before bringing in, transporting for entry or using nuclear material or those components containing such material, contact the CEPM to ensure the correct JBSA entities are aware and engaged to support the Contractor's success.

a. Non-Air Force agencies, including contractors, who intend to use, or store, radiation sources on Joint Base San Antonio, Fort Sam Houston and Camp Bullis must obtain proper authorization in accordance with the above references AFI 40-201 and AFI 48-148. Non-Air Force agencies will not use radiation sources on referenced locations without an approved Department of the Air Force Radiation Permit. Such equipment typically includes equipment for Soil Density Testing, Lead-Based Paint Analysis (X-Ray fluorescent analyzer), etc.

b. Non-Air Force agencies (including civilian contractors) require an Air

Force Radiation Permit(s)(AFRP) to use or possess ionizing radiation sources on an Air Force installation. For the purpose of this paragraph, ionizing radiation source means any source that, if held or owned by an Air Force organization, would require a specific NRC license or Air Force radiation authorization - ARA.

c. The non-Air Force applicant will apply by letter with supporting documentation through the Installation Radiation Safety Officer (RSO).

Submit the letter so that the installation representative receives the application at least thirty (30) days before the requested start date of the permit.

d. The Air Force radiation permit (AFRP) application will specify start and stop dates for the AFRP and describe for what purposes the applicant needs the AFRP. See the document in Appendix A for AFRP requirements and instructions to apply for a permit. The following information shall be submitted to the RSO for the AFRP request.

1) Brief Description of Proposed Activities

2) The applicant possesses a valid U.S. Nuclear Regulatory Commission

(NRC) Form 347 material license that allows the applicant to use the source as specified in the AFRP application.

3) The applicant possesses a valid Agreement State or Radioactive

Material License (Texas Department of Health, Bureau of Radiation Control) license that allows the applicant to use radioactive material (RAM) as specified in the AFRP application and the applicant has filed NRC Form 241, Report of Proposed Activities in Non-Agreement States, with the NRC in accordance with 10 CFR

150.20. A copy of the NRC Rom 241, must accompany the TRC Form 12-1. An AFRP issued under this circumstance will be valid for no more than 180 days in any calendar year.

4) Project Local Point of Contact: Name, Address, and Phone.

5) Company Radiation Safety Officer: Name, Address, and Phone.

6) Copy of Contractual Clause of the Air Force/Army Contract describing work to be performed and inclusive dates (from the contract)

7) Provide log of actual equipment usage to the JBSA-FSH RSO.

8) Leak test documentation for the equipment with the radioactive source(s).

9) Site map defining area where radioactive materials/equipment is to be used.

10) Radiation Safety Program to include standard operating procedures (SOP), training program and designation of Radiation Protection/Safety Officer.

11) Written confirmation that the JBSA-FSH RSO can conduct periodic assessments and has authority to suspend contractor operations believed to be unsafe.

12) For naturally occurring or accelerator produce radioactive material (NORM/NARM) and machine-produced ionizing radiation sources, the applicant has an appropriate State authorization that allows the applicant to use the source as specified in the AFRP application or has in place a radiation safety program that complies with Air Force requirements.

e. All AFRPs will require applicants to remove all permitted sources from Air Force property by the end of the work day. (Ft. Sam Houston policy, no radiation producing equipment will be stored on Ft. Sam Houston and Camp Bullis property overnight).

f. Disposal of radioactive material (RAM) by non-Air Force agencies on Air

Force property is strictly prohibited. 3.2.3 Review, Approval/Disapproval. Review of the permit request and subsequent approval/disapproval requires approximately 30 days.

g. Submission of Application. Submit three copies of the application with the required supporting documentation to 502d Air Base Wing Safety Office, Safety Point of Contact:

1.4.4 Work Outside Regular Hours

Work outside regular working hours requires Contracting Officer approval.

Interruptions and outages to the Chill water loop shall be conducted after hours, weekends and/or when the Dining facility is closed. The Contractor shall work closely with the 802 CE Inspector (802D CEI) and the CEPM to makes arrangements. The CEPM and CEI will advocate on behalf of the Contractor. Make application 14 calendar days prior to such work to allow arrangements to be made by the Government for inspecting the work in progress, giving the specific dates, hours, location, type of work to be performed, contract number and project title.

1.4.5 Permits and Requests

All permit requests must include at minimum the following:

a. Site Map (8" x 11" or 8" x 14") 2 complete hard copies

b. Construction Drawings 5 complete copies

c. Project Name and Number

d. Contractor & Subcontractor Name

e. Brief Description of Work - Identify all buildings and/or facilities possibly affected.

f. Desired Date for Work to Begin

g. Estimated Duration of Work

h. 14 Day Advance Notice minimum, unless otherwise noted.

1.4.5.1 Digging Permits / Utility Locate

a. Digging permits must be obtained prior to prior to any digging, drilling or any excavation work to include post driving.

b. Digging permit /utility locates are available by submitting the completed US Air Force Form 103 Base Civil Engineer Clearance Request which will be provided to the Contractor at time of Notice To Proceed by the CEPM or the Civil Engineer's Inspector CEI. Contractor shall also register with and contact TX 811 (https://www.texas811.org/ )IAW Title 5. Texas Administrative Code. Contractor shall Contact the San Antonio Water System and request a locate service.

c. Contractor shall attend the Utility Locate meeting held each Wednesday at 1300hrs in B7065, JBSA-LAK for one stop-shop Government review of digging permit requests. A digging permit for a specified area of excavation expires 30 days after the issue date; the contractor shall re-apply for a new permit to perform excavation in the area if the excavation was not started within the 30-day period.

d. Permits will identify all underground utilities within 5 feet of the designated area. Contractor shall be responsible for all repairs, costs, and damages due to excavating without permit or damaging an identified utility. The Contractor shall maintain the Government markings until the excavations are complete (severe weather is an exception). The Contractor shall be responsible to the Government for a $75.00 remarking fee if the Government is requested to remark.

1.4.5.2 Utility Outage Permit

a. Contractor shall complete and submit a written request 30 days in advance of the requested utility outage to CEPM and CEI. The request shall include the utility service to be affected, the proposed date, and two proposed alternate dates, proposed time, and outage duration.

b. The Contractor shall be responsible to coordinate with CPS Energy about any energy or gas outage request and San Antonio Water System (SAWS) for any water, storm water or sanitary sewage interruptions/outages.

This coordination is separate than the government coordination.

The Contractor shall simultaneously notify in writing the CEPM and CEI so that they can monitor the process with our public private Utility Corporation Colleagues and intercede on the Contractor's behalf to help prevent unnecessary delays and confusion.

c. Provide CEPM and CEI a copy of the signed written Utility Outage request prior to any outage.

d. After submitting the request to the government, the contractor shall receive approval from the government prior to schedule a utility outage.

1.4.5.3 Hot Work Permits

Under no circumstances shall hot work operations start without a hot work permit.

All welding and cutting operations shall be performed in accordance with National Fire Protection Association NFPA Standard 5lB Fire Prevention during Welding, Cutting, and Other Hot Work. Contact the site specific JBSA fire department to request an AF Form 592 USAF Welding, Cutting and Brazing Permit. Ensure a copy of are:

Permits expire in 30 days. If work will extend beyond 30 day period the contractor shall contact JBSA Fire and Emergency Services to re-issue a new permit.

1.4.5.4 Road Closure Approval Requests

a. Road closings will not be permitted without approval. To get approval for a proposed road closure contractor shall submit a traffic control plan to the 802D CES Project Manager prior to anticipated need for closure

b. The Contractor traffic control plan will include each section of road that will be worked on or impacted as a result of this contract. The traffic control plan shall be in accordance with "Traffic Controls for Street and Highway Construction and Maintenance Operations" of the latest edition of the "Texas Manual on Uniform Traffic Control Devices."

c. Contractor will provide traffic control signs and barricades for all street widening and extensions. At least one lane of traffic must be maintained at all times.

1.4.6 Occupied and Existing Building[s]

The Contractor shall be working in and around existing buildings, which are occupied. Do not enter buildings other than B10056 without prior approval of the Contracting Officer.

The existing building and its contents must be kept secure at all times. Provide temporary closures as required to maintain security as directed by the Contracting Officer.

Provide dust covers or protective enclosures to protect existing work that remains and Government material located in the facility during the construction period.

Leave remaining attached equipment in place, and protect it against damage, or temporarily disconnect, relocate, protect, and reinstall it at the proper time based on the Contractor’s execution of work features or at the completion of the work whichever is soonest.

1.2 SUBMITTALS

Government approval is required for submittals with a "G" designation;

submittals not having a "G" designation are for Contractor QC approval.

Submit all necessary submittals using the format below, as indicated throughout this specification and as indicated on the Draft Submittal register appendix. The following is in accordance with this section.

Photographically document site conditions prior to start of work, during work, prior to cover up of permanently concealed work, and at completion of work. Color digital photographs, in JPEG format, shall be provided to the Contracting Officer with each progress payment for unrestricted use by the Government.

1.5 CONTRACTOR SPECIAL REQUIREMENTS

1.5.1 Asbestos Containing Material

Prior to conducting any demolition activities, shall test for both Lead and asbestos of the entire Chiller Plant. This test shall include sampling out to the Cooling Towers. No HAZMAT is expected, however the Government does not have a current test available. The Contractor shall conduct the tests IAW TX law and the Environmental Section of these Specifications and provide a copy of these reports to the CE Project Manager Prior to conducting any other work. Testing shall be accomplished by TX Licensed companies and laboratories.

1.5.2 HVAC TAB, and Apparatus Inspection

The Contractor should refer to UFGS Section 23 05 93 TESTING, ADJUSTING AND BALANCING FOR HVAC and the Testing Organizations requirements manual for further guidance and understanding of TAB procedures. The TAB Agent shall be chosen early but no later than 10 days from the issuance of the Notice to Proceed. This is to ensure the TAB Agent is involved and can guide the decision making processes.

1.6 SUPERVISION

1.6.1 Minimum Communication Requirements

Have at least one qualified superintendent, or competent alternate, capable of reading, writing, and conversing fluently in the English language, on the job-site at all times during the performance of contract work. In addition, if a Quality Control (QC) representative is required on the contract, then that individual must also have fluent English communication skills.

1.6.2 Superintendent Qualifications

The project superintendent must have experience in construction on HVAC projects similar in size and complexity. The individual must be familiar with the requirements of EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. The individual must be capable of interpreting project specifications, project schedule, and construction drawings. The qualification requirements for the alternate superintendent are the same as for the project superintendent. The Contracting Officer may request proof of the superintendent's qualifications at any point in the project if the performance of the superintendent is in question.

For this project the superintendent is permitted to also serve as the Quality Control (QC) Manager IF the individual also has the QC Qualifications required by this Specification. Quality Control of work to include submittals shall not suffer if the same individual handles both roles.

1.6.2.1 Duties

The project superintendent is primarily responsible for managing and coordinating day-to-day production and schedule adherence on the project.

The superintendent is required to attend meetings that may arise and be scheduled i.e. JBSA Red Zone meetings, partnering meetings, and quality control meetings. The superintendent or qualified alternative must be on-site at all times during the performance of this contract until the work is completed and accepted.

1.7 PRECONSTRUCTION

CONFERENCE

After award of the contract but prior to commencement of any work at the site, meet with the Contracting Officer to discuss and develop a mutual understanding relative to the administration of the contract, safety program, quality control program, safety, security, and other topics such submittals, scheduling programming, and the Contractor’s intended method of prosecution of the work. Major subcontractors who will engage in the work shall also attend.

1.7.1 Start of Construction Work

If the Contractor has submitted the Accident Prevention (Safety) Plan, Quality Control Plan, and Environmental Protection Plan for review prior to this meeting, these may be accepted in to or accepted with comments at the conference. Construction work will not proceed until after this meeting has been held, the plans noted above have been accepted, and the Notice to Proceed has been received and acknowledged by the Contractor.

1.8 PARTNERING

To most effectively accomplish this contract, the Government requires the formation of a cohesive partnership within the Project Team whose members are from the Government, the Contractor and their Subcontractors. Key Stakeholders from the Installation, the Contractor and Subcontractors, and the Designer of Record will be invited to participate in the Partnering process. The Partnership will draw on the strength of each organization in an effort to achieve a project that is without any safety mishaps, conforms to the Contract, and stays within budget and on schedule.

The Initial Partnering session should be a part of the Pre-Construction Meeting. Partnering sessions will be held at a location agreed to by the Contracting Officer and the Contractor. The Partners will determine the frequency of the follow-on sessions.

1.9 AVAILABILITY OF CADD DRAWING FILES
