STATEMENT OF WORK
C.1 STATEMENT OF WORK
GENERAL INTENTION: The intention of this solicitation is to obtain operation, maintenance, service and repair for the collection, on-site mobile shred, and recycle of scrap paper generated at the Andrew T. McNamara
Headquarters Complex (HQC). The contractor shall furnish all labor, tools, materials, equipment, transportation and supervision, except as specified herein as Government-furnished, to manage and perform all operations for the collection, on-site shred, and recycle of scrap paper generated at the Andrew T. McNamara Headquarters Building, Building 2462, the Child Development Center (CDC), Building 2468, and the Material Receiving and Screening
Facility (MRSF) located at 8725 John J. Kingman Road, Fort Belvoir, VA 22060. All buildings are located within the fence line of the HQC. The HQC contains over 1.13 million square feet of administrative space supporting over
6,300 employees.
C.1.1 GENERAL REQUIREMENTS. The contractor shall be required to collect paper products from two locations within the HQC. Using mobile shred trucks, shred all paper to maximum size of 0.625 inch by 2 inch on site, provide weight of paper shred with certificate of destruction prior to leaving, remove from the installation and recycle. The contractor shall use Government provided collection containers.
At building 2462, the contractor is to retrieve the containers from Room 4104 within the building, twice per week for on-site shred prior to removal for recycling.
At building 2468, the contractor is to retrieve one container from the exterior of the building twice per week for on-site shred prior to removal for recycling.
On occasion, there may be a requirement for additional pick-ups due to high volume. Such requirements will be handled on an on-call basis.
There are areas within the building that require documents to be shred prior to leaving the office. The bags of shredded documents will be taken by others to the recycle room 4104. This pre-shredded material shall be picked up and removed to be recycled as an on-call service.
C.1.1.1 Performance shall be accomplished according to the terms, conditions and standards contained herein and current local, State and Federal regulations. The contract shall be firm fixed price to be effective for a base year from date of award for one year, with four (4) sequential one (1) year option years.
C.1.2 PERSONNEL.
C.1.2.1 Project Manager. The contractor shall provide a Project Manager (PM) to act as the contractor's representative, who shall be responsible for the performance of the work under this contract. The name of this person and alternate(s) who can act for the contractor when the PM is absent shall be designated in writing to the Contracting Officer (KO).
C.1.2.1.1 The PM shall have full authority to act for the contractor at the HQC.
C.1.2.1.2 The PM or alternate shall be available by telephone at all times during normal duty hours, 0700 to 1700, Monday-Friday on regular Government workdays (Federal holidays excepted). This representative will direct the contractor's working force and operations in accordance with the requirements herein. The PM shall be empowered with sufficient authority to enable him to meet conditions which arise in the day-to-day operations without undue delay. The contractor shall also provide a point-of-contact and phone number to be called after duty hours in case of emergency.
C.1.2.1.3 Complaints received by the PM from the Contracting Officer's Representative (COR) or HQC Installation
Management Office prior to 0930 hours of work not done or not done in accordance with contract requirements, during that workday or the previous workday, shall be corrected prior to 1500 hours on the day the complaint is received, at no additional cost to the Government. The PM shall also be responsible, when deficiencies have been corrected, for reporting the corrected deficiencies to the COR.
C.1.2.1.4 The PM shall properly acknowledge the receipt of Quality Deficiency Reports (QDR). When reported deficiencies have been corrected, the PM shall be required to provide the QDR (fax copy or hand carry copy) to the COR, who will then certify that the deficiencies were or were not corrected timely. If the contractor does not provide the required QDR, the COR will take a deduction on the contractor’s monthly invoice in accordance with
C.3.2.1, Monetary Adjustment for Inadequate Performance.
C.1.2.2 Contractor’s Employees. The contractor shall furnish sufficient personnel to perform all work specified within the contract. The contractor's employees shall maintain themselves and their clothing in a neat and clean manner and shall conduct themselves in a proper and efficient manner at all times.
C.1.2.2.1 The Contractor's employees shall be citizens of the United States of America, or aliens who have been lawfully admitted for permanent residence or who present other evidence from the Immigration and Naturalization
Service that they may lawfully be employed within the United States.
C.1.3 POST AWARD CONFERENCE. Prior to commencing work, the contractor shall attend a post-award conference with Government representatives, on a date and time to be arranged by the Contracting Officer (KO), to discuss and develop mutual understandings relative to the work schedule and administration of work in accordance with the Statement of Work, Section C.
C.1.4 SITE VISIT. A site visit will be scheduled prior to award of the contract, a date and time convenient to both the contractor and the Government.
C.2 DEFINITIONS. As used throughout this contract, the following terms and phrases apply as defined:
C.2.1 Collection Station. Also referred to as a collection point, pick-up station, or collection site, is a designated point where paper will be collected by the contractor.
C.2.2 Collection Container 100% recyclable 64 gallon mobile plastic recycle container with reinforced locking lid.
C.2.3 Contracting Officer (KO). A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the KO acting within the limits of their authority as delegated by the KO.
C.2.4 Contracting Officer’s Representative (COR). The person whom the Contracting Officer will designate by name or position title to act as intermediary between the Contractor and the Contracting Officer on matters pertinent to this contract and be his/her authorized representative.
C.2.5 Alternate Contracting Officer’s Representative (ACOR). The person whom the Contracting Officer will designate by name or position title to act as intermediary between the contractor and KO on matters pertinent to this contract and be his/her authorized representative when the COR is not available. The ACOR is equal to an alternate
COR in terms of any section of work as required by these specifications.
C.2.6 Deficiency. The quality or state of lacking in some necessary standard or element in terms of any section of work as required by these specifications
C.2.7 Mobile Shredding Truck. Also referred to as mobile document shredding truck. Truck with inside mounted industrial shredder capable of shredding 5,000 pounds per hour to a maximum cross-cut shred size of 0.625 inches by 2 inches. Truck must be equipped with automated cart tipper to eliminate contractor contact with paper waste, internal camera with external monitor for monitoring of shred process, and scale with print capability to provide weight of materials at time of shred.
C.2.8 On-Call. A method used by the Contracting Officer’s Representative or his/her alternate to contact the contractor for a service within the statement of work and a timeframe for the delivery of that service.
C.2.9 Quality Control (QC). A method used by the contractor to control quality of goods and/or services produced.
C.2.10 Quality Assurance (QA). A method used by the Government to provide some measure of control over the quality of purchased goods and/or services received. The Government’s Quality Assurance Surveillance Program is not a substitute for quality control by the contractor.
C.2.11 Quality Deficiency Report (QDR). A document issued by the Government identifying the contractor’s unsatisfactory performance.
C.2.12 Recyclable Waste. Waste material that can be transformed into new products in such a manner that the original product may lose its identity. Many of the components of our waste stream, from metals and plastics to used oil and paper, are recyclable waste; recyclables other than scrap paper shall be maintained by the Fort Belvoir
Recycle Center, and are not included under this contract.
C.2.13 Scrap Paper. Paper suitable for recycling. Paper collected from the HQC may include, but is not limited to:
white paper, colored paper, blueprints, telephone books, magazines, and envelopes.
C.2.14 Spillage. Paper or pulp dislodged from collection containers, shred trucks or other equipment used during the course of collection, shredding, and/or recycle operations under this contract.
C.2.15 Vector. An organism capable of carrying pathogens (agents that cause disease) from one organism to another.
C.3 SURVEILLANCE.
C.3.1 QUALITY ASSURANCE. The Government will monitor the contractor's performance under this contract using the quality assurance procedures in the Surveillance Plan, Section C.3.2.
C.3.1.1 Performance Evaluation Meetings. The contractor's PM shall meet at the HQC, Building 2462, with the
COR and the KO during the first month of the contract, on a date and time to be arranged by the KO. Performance evaluation meetings shall also be held each time a Quality Deficiency Report (QDR) is issued or as requested by the
COR. Additional meetings shall be held thereafter as often as the KO determines necessary. A mutual effort will be made to resolve all problems identified. The Contractor shall draft the meeting minutes and submit to the COR for review. The written minutes of these meetings will be signed by the PM, KO, and COR. Should the contractor not concur with the minutes, the contractor shall prepare a written response within two (2) working days of receiving the meeting minutes, stating to the KO any areas wherein the contractor does not concur.
C.3.2 SURVEILLANCE PLAN. All work and services performed as included in the Statement of Work for this contract shall be done in accordance with proper collection, shredding and recycling procedures under the terms of this contract. All contractor provided work and services shall be subject to Government inspection and approval, with up to one-hundred (100) percent sampling for each requirement at the option of the Government. The acceptable quality level (AQL) of these specifications and requirements is one-hundred (100) percent.
SURVEILLANCE ACTIVITY CHECKLIST
Safety and Reports
The following checklist is for items which do not require 100% sampling. The items contained on the Checklist and the frequencies of inspections are as follows:

C.3.2.1 Monetary Adjustments for Inadequate Performance. Nonperformance or failure to satisfactorily accomplish work in accordance with the provisions of this contract, due to the fault of the contractor, shall constitute a deficiency under this contract. All work will be regularly inspected by the COR, and deficiencies will be reported in writing to the contractor. If deficiencies are not corrected in a satisfactory and timely manner monetary deductions will be made on invoices submitted by the contractor. For all fixed-price work, Schedule B, Supplies and Services, monetary deductions will be made from the line items on the applicable monthly invoice(s) for the services, and if necessary, on succeeding monthly invoices. For all on-call work, monetary deductions may be made on the invoice(s) for that work.
C.3.2.1.1 Service Deficiencies: Failure to perform the required services on the days indicated will constitute a deficiency and a QDR will be issued by the COR. The contractor shall take corrective action and correct any deficiencies promptly after notification from the COR. The Contractor shall be required to provide the QDR (fax copy or hand carry copy) to the COR indicating how the discrepancy occurred, how the discrepancy was corrected and how the Contractor plan to keep the discrepancy from happening in the future. Deductions from the appropriate monthly invoices for deficiencies will be made as follows:
C.3.2.1.1.1 Failure to collect, empty and return all collection containers by 1530 hours on the date scheduled may result in a 10 percent deduction of the daily price for each day of nonperformance.
C.3.2.1.1.2 Failure to pick up spillage in the vicinity of the shred site may result in a 10 percent deduction.
C.3.2.1.2 Equipment Deficiencies
C.3.2.1.2.2 Government provided collection containers shall be in serviceable condition. The Contractor shall notify the Government of containers deemed unserviceable.
C.3.2.1.3 The COR is delegated the authority to make determinations as to the existence of a deficiency in performance, and the timeliness to correct a deficiency. The KO will be provided copies of all deficiency notices issued by the COR.
C.3.2.1.4 If the contractor does not comply with all provisions specified herein, the Government reserves the right to correct the deficiency and charge the contractor for all costs incurred.
C.4 GOVERNMENT-FURNISHED PROPERTY AND SERVICES.
C.4.1 GOVERNMENT-FURNISHED EQUIPMENT.
C.4.1.1 The Government will provide 64 gallon secure document bins with locking lids. Contractor shall remove bins from pick up locations, shred contents on site and return bins to pick up location.
C.4.2 SPACE FOR CONTRACTOR'S OPERATION.
C.4.2.1 The Government will provide one interior location for storage of collection containers at building 2462, Room 4104 and an exterior space at building 2468 for storage of a single collection container.
C.4.2.2 Sufficient clearance for access to loading dock is provided.
C.4.2.3 The contractor shall make no alterations to the space provided without written permission from the COR.
C.4.2.4 At contract completion the contractor shall vacate such space and restore the premises to the condition in which received at its expense, fair wear and tear excepted. The contractor shall be held responsible for the cost of any repairs caused by negligence or abuse on its part, or on the part of its employees.
C.5 CONTRACTOR-FURNISHED ITEMS. It is the responsibility of the contractor to furnish all items necessary to meet the requirements of this contract. Failure to furnish equipment in the quantity and of the type and quality necessary to perform the prescribed work in accordance with the provisions of this contract shall be cause for termination pursuant to the "Default" clause of the contract. Experience has shown that at least the following collection equipment, fully operational and exclusive of that dead lined for maintenance or repair, is necessary to accomplish the work called for by the contract:
a. Mobile Shred Truck
b. Box Truck for pre-shredded bag pick ups
C.5.1 MOBILE SHRED TRUCKS AND BOX TRUCKS.
C.5.1.1 All mobile shred trucks and box trucks shall be specifically designed for the purpose; and shall be compatible with all other collection equipment provided the Government and by the contractor. Mobile Shred Truck shall be provided with automated cart lift, shred monitoring capability, and printed weight report following collection. Mobile Shred Truck shall be capable of processing a minimum of 5,000 pounds per hour, which may require multiple visits during a single day to process all materials.
C.5.1.2 All mobile shred trucks and box trucks shall present a neat and clean appearance, be in fully operable condition, and meet all local, State, and Federal safety and environmental protection requirements, including those in Title 40 CFR, Part 243. Equipment found to be in poor operating condition or otherwise to be deficient by inspection of the COR, and reported to the KO shall be immediately removed from service and replaced at the contractor's expense.
C.5.1.3 Mobile shred trucks and box trucks shall be maintained in a manner to prevent leakage of any fluids. Such leaks have potential to pose harm to the environment, and as such, must be responded to in an urgent manner. Any such leak identified by the contractor must be immediately contained and reported to the COR as soon as possible.
The COR will take action to notify the HQC Safety office or Fort Belvoir Environmental Office as appropriate. The contractor is responsible for all site clean-up, as well as any cost incurred should the fluid reach the storm water drainage system. If the contractor is not on site when the leakage is identified, the COR will notify the PM and proceed with containment; the contractor will then have one hour to arrive onsite to proceed with clean up. If the equipment is deemed unserviceable it must be removed in accordance with C.5.1.4
C.5.1.4 All mobile shred trucks and box trucks which become inoperative or break down shall be removed from the
HQC within four (4) hours.
C.5.1.5 All mobile shred trucks and box trucks shall be operated in a safe manner in compliance with Federal, state, local, and installation regulations.
C.5.1.6 All mobile shred trucks and box trucks to be operated at Fort Belvoir are required to display a valid mechanical safety inspection sticker. If the State in which the vehicle is licensed or registered does not require a mechanical safety inspection, the vehicle shall meet the safety requirements of the State of Virginia.
C.5.1.7 All mobile shred trucks and box trucks operated at Fort Belvoir are subject to AR 190-5, copies of which are available in the Fort Belvoir Provost Marshal Office.
C.5.2 COLLECTION CONTAINER.
C.5.2.1 Collection containers shall be provided by the Government for all paper shredding service under this contract.
C.5.2.2 Each Government provided collection container will be 64 gallon capacity, wheeled with castors, hinged lids with integrated locks and constructed of recycled plastic. Collection containers will be Greystone in color.
C.5.2.3 Collection containers shall be deemed unserviceable if any wheel condition prevents the container from rolling smoothly, if the lid cannot be attached or locked, or if a side wall fails causing the container to gap open. The
Contractor will notify the Government of any unserviceable containers
C.6 SPECIFIC TASKS TO BE PERFORMED.
C.6.1 Scrap Paper Collection, Shred and Recycle.
C.6.1.1 At building 2468, the CDC, the collection container stored outside the building shall be emptied and shredded to a size no larger than 0.625 inch by 2 inch. Within building 2462, collection containers shall be retrieved from designated locations, taken to the loading dock and shredded to a size no larger than 0.625 inch by 2 inch. Collection container at building 2468 shall be serviced prior to building 2462. A separate notice of destruction and weight of shred collected shall be provided to the COR for each building prior to departure from building 2462. Regularly scheduled services must be provided between 0630 and 1530 Tuesday and Thursday. On call services shall be scheduled through the COR. Paper pulp collected in the truck shall be removed from the installation for recycle by the contractor and processed in accordance with all Federal, State and local laws.
C.6.1.2 Federal Holidays. The contractor shall not collect scrap paper on Federal holidays unless otherwise directed to do so by the KO. Federal holidays are:

Note: When one of the above holidays falls on a Sunday, the following Monday will be observed as a legal holiday.
When a Federal holiday falls on a Saturday, the preceding Friday is observed as a legal holiday. If the day scheduled for normal pickup falls on a legally observed Federal holiday, the contractor shall conduct the pickup on the next Federal workday, or on an alternate day agreeable to the COR.
C.6.1.3 Times of Collections. All pickups shall be made between 0630 and 1530 hours on regular Government workdays, including on-call pickups.
C.6.1.4 Aesthetics. Scrap paper shredding operations shall be conducted in an aesthetically acceptable manner at all times as determined by the COR. Blowing litter shall be controlled and picked up. Policing of the work area shall be accomplished by contractor personnel at the same time as each loading, which shall be performed so as to minimize the scattering of paper or pulp. Also see, Sanitation Requirements, Section C.6.6.
C.6.2 SCHEDULE AND POINTS OF COLLECTION. The contractor shall provide necessary services for collection, shredding and recycle as described below.
C.6.2.1 Route and Collection Schedules.
C.6.2. 1.1 The contractor shall establish vehicle routes for the duration of the contract; and collection schedules for the full performance period and submit these routes and schedules to the KO for approval ten (10) calendar days after contract award and sixty (60) calendar days prior to each option year being exercised. The contractor shall submit revised routes to the COR for review and approval by the KO before changing any established route.
C.6.2.1.2 The collection schedule shall be established so collection operations shall be made between 0630 and 1530 hours on regular Government workdays (Federal holidays excepted). Collection operations outside these hours will require prior approval of the COR. Pickups shall be on the same day or days of the week, as indicated on the approved schedule for the duration of the contract.
C.6.2.2 Collection Sites / Container Locations.
C.6.2.2.1 The Government reserves the right to direct the contractor to relocate any or all collection sites. In the event the COR determines that a relocation is necessary, the contractor shall adjust its services to meet such relocations without additional expense to the Government.
C.6.2.2.2 The contractor shall return empty containers to their proper collection stations after servicing in an upright position with the lids securely in place.
C.6.3 UNSERVICEABLE CONTAINERS. The Contractor shall notify the Government of any unserviceable containers. The Government will remove and repair or replace with serviceable containers.
C.6.4 HEADQUARTERS COMPLEX CLOSURES OR REDUCED OCCUPANCY The Government reserves the right to reduce services under the following conditions:
C.6.4.1 When the Government announces a closure over local radio or television stations due to unusually severe weather
C.6.4.2 When the HQC is open and operating at a reduced occupancy of less than 50%.
C.6.4.3 When an unforeseen closure or reduced occupancy of the facility occurs on a regularly scheduled day of work, the Government shall have the following options:
C.6.4.3.1 To require the Contractor to perform the work on the following day unless the following day is a Saturday or Sunday.
C.6.4.3.2 To forego the work and reduce payment due the contractor accordingly (on a pro rata basis) for work not performed.
C.6.4.3.3 To switch regularly scheduled services to an on call basis.
C.6.6 SANITATION REQUIREMENTS. The contractor shall provide the services as described below:
C.6.6.1 The contractor shall be responsible for picking up and disposing of all recyclable paper which is within fifteen (15) feet of collection points; and for immediately picking up all spillage that occurs in the course of collection and/or shredding operations.
C.6.6.2 The equipment shall be thoroughly steam cleaned, chemically treated, or washed with soap or detergents and water as specified below. Other methods of cleaning shall require approval of the COR.
C.6.6.2.1 Shred trucks, including the bodies, shall be washed as required with soap or detergents and water so as to maintain a clean, neat condition and to prevent the propagation or attraction of vectors. See Section C.3.2 for deliverable required for submission to the COR.
C.6.6.2.2 Collection containers shall be inspected for clean condition and proper operation. The contractor shall inspect containers during each shred collection for condition and operation. Any containers found in need of cleaning or repair shall be reported to the COR the same day of service.
C.6.7 APPARENT SERVICEABLE MATERIAL. The contractor shall not dispose of any material, which has obvious value and appears to have been inadvertently placed near the collection point, unless such material is designated as recyclable waste by the COR. The COR or HQC Installation Management Office shall be notified of any such material found by contractor personnel.
C.6.8 DISPOSAL. The contractor shall recycle all shredded materials at an off-post recycle facility permitted by the
Commonwealth of Virginia. The proposed facility shall be in compliance with all Federal, State, and local laws and regulations and have no outstanding notices of violation at the time of award or during the Period of Performance.
The contractor shall be responsible for any permit or fees associated with the use of off-base recycle locations.
C.6.9 DAILY COLLECTION DATA REPORT. The contractor shall furnish the COR daily, prior to departure from the site, the weight of materials collected for shred and recycle. If multiple loads are required, the contractor shall furnish an individual report for each load. The scale must be calibrated monthly at a minimum to ensure accurate weight is provided.
C.6.10 MONTHLY COLLECTION DATA REPORT. The contractor shall furnish the COR monthly collection data and other data as may be required, attached to the monthly invoice. The reports will show the number of loads, the number of cubic yards per load, and tons of recycle waste per load on the shred truck prior to final disposal. The invoice will be certified by the COR and the data reports will be kept on file by the Government. One copy of each report will be retained by the contractor and the original will be retained by the COR.
C.6.11 ACCIDENT PREVENTION.
C.6.11.1 In order to provide safety controls for protecting the life and health of employees and other persons; for preventing damage to property, materials, supplies, and equipment; and to avoid work interruptions in the performance of this contract, the contractor shall comply with all pertinent local, State, and Federal safety requirements, and shall also take whatever corrective action the COR deems necessary.
C.6.11.2 The contractor shall maintain an accurate record of any accidents that occur under this contract. The contractor is required to provide an immediate verbal report to the COR and follow-up within twenty-four (24) hours with a written report to the COR of any such occurrence. The COR will provide a copy of any accident report received from the contractor to the Government’s Safety and Health Office. In addition the contractor shall report to the COR, exposure data on all accidents resulting in death, traumatic injury, occupational disease, and/or damage under this contract.
C.6.11.3 If the COR notifies the contractor of any noncompliance with the foregoing provisions, the contractor shall, after receipt of notice, immediately take corrective action. Such notice when delivered to the Project Manager or alternate at the work site shall be deemed sufficient for this purpose. If the contractor fails or refuses to comply within twenty-four (24) hours, the COR may reduce contract payments from invoices.
C.7 DELIVERABLES.
C.7.1 The contractor shall provide the following reports or documentation to the COR on the dates indicated according to the cited reference:

C.7.2 In addition to the records required above, upon notification from the KO, the contractor shall provide the KO with reports relating to labor statistics or other data as requested, within five (5) working days.
C.8 SECURITY REQUIREMENTS.
C.8.1 Building Security. The Contractor shall be responsible for safeguarding all Government property provided for Contractor use. At the close of each work period, Government facilities, equipment and materials shall be secured.
C.8.2 Entrance into the HQC is controlled through a single entry point. Visitors (personnel not part of the HQC workforce, Government or Contractor) are required to sign-in and sign-out at the main entrance Atrium Guard Desk.
Permanent workforce personnel with DLA HQC badges are not required to sign-in or sign-out. All Contractor personnel are required to check-in and check-out between 7:00 AM and 4:00 PM with the Contracting Officer’s
Representative (COR). Entrance into the Child Development Center and sections of the HQC have more secure areas and escorts will be provided on days the contractor is providing service.
C.8.3 Contractor Background Security Checks. All contractor employees who work at the HQC shall be required to fill out the government's online security background check, Standard Form 86/85P, National Agency Check with
Inquiries (NACI), and to be fingerprinted in order to obtain a HQC Building Identification Badge. The contractor shall fill out a DLAH 1728 and bring to the COR for signature, then submit to the Personnel Security Office to start the process. The Personnel Security Office will email instructions to fill out the online form. The contractor shall be required to initiate through the internet, an Electronic Questionnaires for Investigations Processing (e-QIP) system form, which is the automated Standard Form 86/85P. The Contractor will be emailed instructions to access the e-
QIP program by the Personnel Security Office and can obtain assistance from the Personnel Security Specialist. The e-QIP signature page and all required documents shall be printed, signed and submitted to the Personnel Security
Office after completion.
C.8.4 The Project Manager and contractor personnel must receive a favorable background investigation before work can be started at the HQC. An initial background check will be provided from the information written on the
EPSQ. Until notification from the Personnel Security Office, employees will be directed by the security specialist as to whether a further investigation will be required or whether the employee shall sign in and sign out at the main entrance guard desk. Upon notification from the Personnel Security Office of the final and completed background check, the HQC ID Badge will be issued and displayed at all times while working at the HQC. All information placed on the SF 85P shall be true to fact and all information must be filled in and completed.
