About this file

This financial response bid sheet is associated with an Invitation for Bid (IFB) issued by the New York Office of Children and Family Services (OCFS) for the procurement of epinephrine auto-injectors. The procurement seeks approximately 72,000 twin pack auto-injectors in three dosage variants—0.1 mg for children weighing 16.5-33 pounds (24,000 units), 0.15 mg for children weighing 33-66 pounds (24,000 units), and 0.3 mg for children weighing over 66 pounds (24,000 units)—to be delivered to eligible childcare programs throughout New York State. The contract term is two years, and vendors must submit completed bid sheets with unit pricing for each dosage lot along with all required supporting documentation to RFQ@ocfs.ny.gov.

Bidders must provide all-inclusive pricing that covers supply and delivery costs, with a maximum allowable grand total bid price of $15,000,000 for the two-year term. Any bid exceeding this ceiling will be rejected. The procurement carries Minority/Women Business Enterprise (MWBE) contracting goals of 15 percent for Minority Business Enterprises (MBE) and 15 percent for Women Business Enterprises (WBE). The incumbent contractor identified for this procurement is H.F Acquisition Co, LLC, which was awarded an identical or substantially similar contract within the past five years. Vendors must complete all required fields on the financial bid sheet, including vendor identification information, contact details, SFS Vendor ID or Tax Identification Number, and unit bid rates for each lot, with the spreadsheet automatically calculating the grand total cost.
