MANDATORY MINIMUM REQUIREMENTS
Please provide a yes/no (Y/N) in the shaded areas below indicating either your acceptance or rejection of the listed requirement and if you agree to the requirement, you agree to include the documentation in your proposal.
If you respond yes, this means that you have accepted the listed requirement and agree to include the information in your proposal.
If you respond “no” or blank, and you do not supply alternatives as described below, your proposal may be eliminated from consideration.
If an item is left blank, the State will consider this a “no” response indicating that Respondent will not meet the requirement and your proposal may be eliminated from consideration.
This document will be used to score respondents as described in Step 1, Section 3.2, Evaluation Criteria.

If Respondent answers no to any of the minimum requirements listed on this attachment, but believes they have an alternative solution, please answer “No/with alt” in the appropriate box. Please provide Attachment number, Requirement number, and alternative solution with explanation in your proposal. Alternatives will be reviewed and considered by the State as to whether they satisfy the minimum requirements. The State reserves the right to remove your proposal from consideration if the proposed alternative does not meet the needs of the State.

If Respondent does not currently have a CBA with a union, then indicate in Column B above whether you would agree to these terms if during the course of the contract Respondent enters into a CBA with a union.
