About this file

This is a Statement of Work (SOW) attachment for RFS 26-87982 between the Indiana Veterans' Home (IVH) and hospice service providers. The procurement establishes a framework for hospice providers to deliver interdisciplinary palliative care services to eligible residents at IVH, a state-owned long-term care facility in West Lafayette, Indiana. The scope of work encompasses nursing care, physician services, medical social services, counseling (bereavement, dietary, and spiritual), therapy services (physical, respiratory, occupational, and speech), home health aide services, medical supplies, drugs, biologicals, and medical appliances. The hospice provider retains sole authority for patient admission, assessment, discharge, and professional management of care in accordance with Medicare Conditions of Participation and applicable state and federal regulations. Key deliverables include a Care Coordination Plan (due 10 days after contract start), Training Completion Report (due 30 days after start), monthly progress reports, and a Final Transition Package at contract end. The hospice provider must designate a registered nurse available 24/7 for consultation and coordinate care through interdisciplinary team meetings with facility staff.

Payment is structured by service level: for routine services, the hospice provider reimburses the facility 100 percent of the Medicaid daily reimbursement rate for room and board charges; for VA Prevailing Rate residents, the facility provides medications at no charge to the hospice provider and bills VA directly for the daily rate; for General Inpatient Level services, the rate is $659.01 per day (2024 Medicaid Settlement Rate, adjusted annually with FSSA approval); and for Respite Level services, the rate is $700.17 per day (effective 7/1/2026, adjusted annually). The contractor must comply with HIPAA requirements as a Business Associate, implement administrative, physical, and technical safeguards to protect Protected Health Information, report security incidents within specified timeframes (unsuccessful attempts monthly, successful breaches within 2 hours by phone and 24 hours electronically), and maintain access to books and records for Department of Health and Human Services review. The State reserves the right to add Independent Verification and Validation services and may terminate the agreement if the contractor violates HIPAA provisions.
