About this file

This file is a Service Provider Security Assessment Questionnaire (SPSAQ) for the South Carolina Small Software App Development contract opportunity. The questionnaire serves as a mandatory security evaluation tool that prospective contractors must complete and submit with their proposals. Contractors are required to provide detailed responses to eleven comprehensive questions addressing information security practices, disaster recovery capabilities, employee and contractor vetting procedures, third-party contractor management, and relevant security certifications from accredited third parties such as ISO/IEC 27001, AICPA SOC 2 Type 2, or similar compliance documentation.

The questionnaire requires contractors to document their security policies and procedures across multiple critical areas including access control limitations for government information, physical security measures for data centers and hosting facilities, encryption protocols for data at rest and in transit, breach detection controls and audit logging practices, incident response procedures, and post-contract data destruction protocols. Contractors must identify all third parties with access to government information and describe safeguards against unauthorized use, disclosure, or manipulation of sensitive data. The document mandates that an authorized company representative sign and certify the accuracy of all submitted information. Responses may include supporting documentation and attachments as necessary to thoroughly address each requirement.
