Prepare and submit to the owners of the past projects in which you have submitted for Prime/Sub Contractor Experience, a Past Performance Reference Package. For Government contracts, send to the Contracting Officer who awarded the past Government contract.

For commercial references, send to personnel with duties similar to those for Government contracts. Offerors should follow-up and encourage references to get the completed questionnaires to the Contracting Officer via email only to ALANNA_GARDNER@NPS.GOV before the closing date and time of the solicitation. If you have multiple references at one location (Park, etc.), send one cover letter and questionnaire for each contract for which you are requesting a reference. Your questionnaire package should contain the following:

· Cover Letter (See SAMPLE below - page 2 of this document)

· Past Performance Questionnaire (pages 3 and 4 of this document) Offeror must first complete Header Information and complete Sections I & II of the questionnaire in the spaces indicated for identification purposes. The reference must then complete Sections III & IV.

Please Note:

OFFEROR (PRIME CONTRACTOR SUBMITTING PROPOSAL) SHOULD PLACE THEIR COMPANY NAME ON THE TOP RIGHT OF EACH QUESTIONNAIRE PAGE AS THE OFFEROR.

IN THE CASE OF AN OFFEROR PROPOSING TO USE A SUBCONTRACTOR, THE OFFEROR’S (PRIME CONTRACTOR SUBMITTING PROPOSAL) NAME SHALL STILL BE PLACED ON THE TOP RIGHT OF EACH QUESTIONNAIRE PAGE AS THE OFFEROR, AND THE SUBCONTRACTOR’S NAME SHALL BE PLACED IN THE CONTRACTOR NAME SECTION UNDER SECTION I (AND ALL OTHER INFORMATION REQUIRED IN SECTIONS I & II OF THE QUESTIONNAIRE IN THE SPACES INDICATED FOR IDENTIFICATION PURPOSES) BEFORE BEING SENT TO REFERENCES.
