Amendment 002 AG-05G1-S-10-0035 AZURITE MINE ACCESS – ROAD CONSTRUCTION
Azurite Mine Access, Pre-bid site visit, Friday, July 30, 2010
On July 30, 2010, a pre-bid site visit was held for the Azurite Mine Access project. The Federal Business Opportunities webpage can be found by clicking on this link:
https://www.fbo.gov/index?s=opportunity&mode=form&id=d3635202886ea8a983894ebf8c165bf4&tab=core&_cview=0
The following people participated in this site visit:
Forest Service Employees
David Colbert, project engineer.
Rod Lentz, project geologist.
Kathy VanHecke, project bridge engineer.
Laurie Dowie, Methow Valley Ranger District representative.
Contractor Representatives
Pete Delange, Boulder Creek Construction.
Bob Miller, N.A. Degerstrom
Ed Dahlin, Cates and Erb
Mark McMillan, McMillan Construction
Don Maples, Lloyd Logging
Roger Bevier, Tollefson Construction
Bob Tollefson, Tollefson Construction
Jerry Palm, Palm Construction
Brian Vinton, Christen & Associates
John Hansen, Hansen Logging
Bob Lloyd, Lloyd Logging
Hurst Construction (representative’s name not recorded).
The site visit agenda started with all participants meeting at the Methow Valley Ranger District Office in Winthrop, WA at 0900. The group was then led to the project site by Forest Service personnel. Drive time from the Forest Service
District Office to the ATV/dirt bike off-load site took approximately one and a half hours. Once all ATVs/Dirt bikes were offloaded, the group reviewed the 1.4 miles of FS road 5400760 that will be reconditioned prior to the South Fork Slate
Creek Bridge site. In general, the 5400760 road will be reconditioned to provide a minimum travelled way width of 14 feet. Clearing and grubbing will be accomplished per contract drawings, with a clearing and grubbing disposal method of scattering. Several stream crossing were encountered along this first 1.4 miles of road. Several of these crossings were identified as good candidates to utilize clearing timbers a buttresses to support reconditioning of the crossings which are sub-14 foot road travelled way standard. There were also several stream crossing near the bridge site which would be more conducive to having culverts installed.
Once at the bridge site, FS Bridge Engineer Kathy VanHecke reviewed the bridge design and potential bridge placement options. Kathy offered one approach to the ACROW bridge installation. This would involve the construction of the bridge sides, or transoms, on the road prism immediately prior to the bridge placement site, then use a tracked crane to lift and place transoms into place. The successful bidder would need to use care when lifting and placing the transoms so as not to allow then to tip and bend. Transoms measure approximately four or five feet tall by two feet wide. It is likely that
Kathy will redesign the bridge to a 80 foot span, rather than a 70 foot span as originally detailed in the drawings. Bridge will be set upon precast footings. If a crane is used for transom placement, it is likely that the lift will require a 55 foot reach. Transom weights are in the neighborhood of 16,000 lbs.
The existing South Fork Slate Creek bridge is a log stringer bridge. This structure will remain in place during and after the placement of the new bridge. It has supported a live load of 16,000 lbs. earlier in the year. This existing bridge could be used to skid abutment materials and the pre-cast footing for the far side of the new bridge.
The 5400760 road bridge approaches will need to be reconstructed to a 50 foot minimum radius, as well as all other horizontal curves on the 5400760 road. Excess material can be hauled forward to suitable disposal site(s).
Once past mile post 1.4 (South Fork Slate Creek Bridge), the 5400760 road continues up to Cady Pass at Mile Post 5.35.

Amendment 002 AG-05G1-S-10-0035 AZURITE MINE ACCESS – ROAD CONSTRUCTION
From Mile Post 5.35 to Mile Post 6.4, side casting material from the road reconditioning operations will be limited to that amount which does not unduly increase visual cut slope disturbance. Remaining material will need to be end hauled forward to appropriate site(s), such as the use in enlarging switch backs.
The road work ends at the Azurite Mine site, at mile post 10.35.
It will be up to the contractor to determine whether blasting will be necessary, or if ripping or rock hammers would suffice to achieve roadway specifications.
A question arose during the course of the site visit as to whether a protective berm, 2’ high by 2’ wide, will be required on the outside shoulder of the road. Since the site visit, it has been determined that such berms will only be required at switchbacks.
A question arose during the course of the site visit as to whether cleared merchantable timber will be required to be decked. The answer is no. Timber will be scattered or otherwise incorporated into the road reconditioning, such as retaining structures at stream crossings.
Turnouts will be constructed in locations best suited, such as pre-existing wide or flat areas, or in locations where excess material can be utilized. A minimum of 10 turnouts are desired.
Upon arrival at mile post 10.35, adjacent to the Azurite Mine tailings, contractor representatives departed the project area.
Arrival at the Mine tailings site occurred at approximately 16:00. Forest Service departed mine site at approximately
16:30.
