SITE VISIT/PRE-PROPOSAL CONFERENCE

Ponderosa Hall Reconstruction

AG-04GG-S-17-0009

Contract Requirement Overview

Disclaimer: Funds are NOT presently available for this effort. The Government reserves the right to cancel, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an Offeror for any costs.

We are still under a Continuing Resolution Authority (federal budget not yet approved by Congress)

DESCRIPTION OF WORK

Base Project is to reconstruct the roof of Ponderosa Hall. The existing flat membrane roof will be removed and replaced with pre-engineered trusses, sheathing, ice and water shield and standing seam metal roofing. HVAC and mechanical system openings will require extension and relocation. Existing cedar shingle siding will be removed and replaced with “Hardie” shake siding. New siding & trim will be painted.

Optional items for Ponderosa Hall include:

• Replacing and repainting 1 x 4 vertical resawn cedar siding and aged wooden shingles with new cement board panel and shake siding;

• Replacing the east wall sliding glass doors and adjacent windows with a new wall with sliding windows;

• Replacing the existing sleeping room windows with new windows;

• Replacing the existing sleeping room AC wall units with new AC wall units; and installing new metal gutters.

All existing painted surfaces on this building are known to contain lead based paint. They must be treated in accordance with LBP guidelines and work procedures outlined by the Oregon Health Authority for Renovation, Repair and Painting and in OAR 333-070-0075 to 333-070-0160 as well as the Oregon CCB regulations in OAR 812-007-0000 to 812-007-0025 and 812-007-0300 to 812-007-0374. Contractors must possess an Oregon LBPR contractor’s license in order to be considered for this project.

All existing sheetrock within the building is known to contain asbestos. The known ACM’s are encapsulated within existing paint and sealed. However, any disturbance of the ACM’s will require full compliance with all laws and regulations concerning asbestos fibers and construction. Asbestos abatement professionals and procedures may be required for some portions of the work.

Work shall be performed during normal business hours of 7:00 am to 5:30 pm, Monday through Friday.

The building will be occupied by residents until September 16, 2017. Structural work for the building may begin after September 16, 2017.

TECHNICAL, PAST PERFORMANCE, and PRICE

1. Technical Capability

a. Logistics Plan. A narrative description of how the work will be accomplished. Addresses items such as timing, coordination, communication, and flow of work.

b. Project Schedule. A detailed time line of how the work will be completed within allotted contract time.

c. Contractor Quality Control Plan.

d. Proposed equipment to be utilized on the project.

2. Key Personnel

(a) Identify the Key Personnel who will work on the project. (Include information for proposed sub-contractors who are expected to perform more than 20% of the work). Include, at a minimum, the following:

i. Project Manager

ii. Site Superintendent.

iii. Quality Control Manager.

(b) For each individual, provide the following information:

i. Name

ii. Position

iii. Relevant Experience: Each resume shall set forth and provide the proposed person's relevant experience.

3. Past Performance:

(a) Complete the attached “Past and Current Contract Information Sheet” for each related project completed in the last 3 years.

4. Price/Business:

(a) Standard Form 1442 with Blocks 14-20 completed.

(b) Complete prices for all items in Schedule of Items (RFP page 3).

(c) Mark the appropriate box in clause K-3 para (b) Annual Representations and Certifications (page 25-27). Since SAM registration is required for award, block (i) should be marked and if there are any updates to your SAM, they should be reflected in paragraph (d) or preferably, your SAM registration is updated.

All evaluation factors other than cost or price, when combined, are approximately equal to cost or price.

The Government will determine best overall value on the basis of the factors described below. Failure to address all items requested by the Government may be cause to consider proposals non-responsive and not eligible for award.

Proposals will be evaluated on the basis of the following factors:

1. Technical Capability

2. Key Personnel

3. Past Performance

4. Price

Technical Capability, Key Personnel, and Past Performance are considered equal in order of importance

TECHNICAL EVALUATION

Technical Capability will be evaluated to determine whether the offeror demonstrates an understanding and sound approach to meeting the solicitation requirements.

Technical Capability will receive a rating of Does Not Meet, Meets, or Exceeds as defined below.

Does Not Meet: Offeror does not demonstrate an understanding and sound approach to meeting the requirements.

Meets: Offeror demonstrates an understanding and sound approach to meeting the contract requirements.

Exceeds: Offeror demonstrates an understanding and sound approach to meeting the contract requirements AND exceeds the requirements of the solicitation

Key Personnel will be evaluated on the quality and extent of the qualifications of the offeror's proposed personnel submitted as part of the proposal. Personnel will be evaluated on their education/experience to the extent it adds value to accomplishing this project. Key

Personnel will receive a rating of Does Not Meet, Meets, or Exceeds as defined below.

Does Not Meet: Key personnel qualifications ARE NOT adequate to ensure successful contract performance

Meets: Key personnel qualifications ARE adequate to ensure successful contract performance.

Exceeds: Key personnel qualifications ARE adequate to ensure successful contract performance AND will add value to accomplishing this project.

PAST PERFORMANCE EVALUATION

Past Performance will be evaluated on performance under existing and prior contracts of a similar nature. The Government will focus on information that demonstrates quality of performance relative to the size and complexity of the procurement under consideration. References other than those identified by the offeror may be contacted by the Government, with the information received used in the responsibility determination and in evaluation of the offerors's past performance.

Past Performance information will consider recency (within last 3 years), relevancy (similar work to this project) and quality of past performance (based on contacting referenced provided in Past Performance Data Sheet). Past Performance will receive one of the following ratings:

Exceptional Very Good Satisfactory Neutral (no past performance) Marginal Unsatisfactory

PRICE EVALUATION

Price will be evaluated as described in FAR 15.305(a)(1) considering any cost information provided to support pay item prices entered in Section B, Schedule of Items. It is the Government's intent to assess the unit prices to determine reasonableness and offeror's understanding of the work and ability to perform the contract.
