About this file
This is an Affidavit of Eligibility Form for the Opportunity Zone Program administered by the Central Florida Tourism Oversight District in Florida. The program aims to increase bidding and contracting participation of businesses located within designated Qualified Opportunity Zones across Orange, Osceola, Lake, Polk, and Seminole counties. To be eligible, a business must have its main office located within the Opportunity Zones maps of one of these five Florida counties as indicated on the FloridaJobs.org website. The form requires businesses to certify their eligibility by providing their business name, main office address, the designated Opportunity Zone county, and an authorized signature. This affidavit should be submitted when responding to a solicitation if the business is not already certified in the program.
The Opportunity Zone Program is a federal initiative created by the Federal Tax Cuts and Jobs Act of 2017 and is designed to foster economic development and job creation in economically distressed communities by encouraging businesses, developers, and financial institutions to invest long-term capital in low-income census tracts. Businesses utilizing this form must confirm that their primary place of business falls within the designated Opportunity Zone boundaries. The related contract opportunity involves mowing and weed control services for rapid infiltration basins, with the requirement that the contractor shall not subcontract any part of the work.
