About this file
This is Addendum No. 5 to the BC-21525-M Pest Management Service Contract Request for Proposal issued by the State of Maryland. The addendum modifies and clarifies the original RFP requirements for pest management services to be provided across designated state facilities and locations. The addendum addresses specific service specifications, treatment protocols, reporting requirements, and performance standards that bidders must incorporate into their proposals. Key modifications include updates to service delivery schedules, pesticide application procedures, documentation and notification processes, and quality assurance measures. All prospective bidders are required to acknowledge receipt of this addendum and incorporate its provisions into their bid submissions by the established due date to ensure proposal compliance and eligibility for contract award consideration.
The addendum establishes revised pricing structures, performance metrics, and operational requirements for the pest management services contract. It clarifies insurance requirements, liability coverage, and contractor certification standards necessary for service delivery. The addendum includes specific environmental and safety compliance mandates, integrated pest management protocols, and emergency response procedures. Bidders must demonstrate capability to meet all amended specifications, and failure to acknowledge and incorporate the addendum revisions may result in proposal rejection. The contract remains subject to all original terms, conditions, and evaluation criteria outlined in the base RFP, with this addendum serving as a binding modification effective upon its issuance.
