About this file

This is a Service Level Agreement (SLA) document for RFP 260000002197 Janitorial Services in Michigan, establishing performance standards and accountability measures for the contractor. The agreement outlines nine specific service requirements with corresponding acceptable standards and financial penalties for non-compliance. Key service areas include emergency response (two-hour callback requirement), correction of unsatisfactory conditions (eight business hours), adequate staffing during designated cleaning times, reporting and safety documentation, equipment and supply provision, snow removal where applicable, and background checks with drug testing for all assigned personnel. The contract is managed by a Program Manager or designee who has authority to assess service credits, determine satisfactory performance, and request corrective actions.

The SLA establishes a financial accountability system through service credits ranging from $25 to $500 per incident, depending on the violation type and frequency. Emergency response failures incur a $250 invoice credit, while unsatisfactory conditions may result in $25 per day initially and $100 per day for subsequent days. Staffing deficiencies are penalized at $250 for the first instance and $500 for subsequent occurrences. Reporting and documentation failures cost $150 per day, and inadequate supplies must be remedied within four business hours or result in a $150 credit on repeat offenses. Background check and drug testing non-compliance incurs $100 per unverified employee per day or a 10% monthly invoice deduction, whichever is greater. The State reserves discretion in applying credits toward contractor payments or direct remittance to the State within ten days of assessment, and may review extenuating circumstances before assessing penalties.
