About this file
This is a Non-Conflict of Interest attestation document required by the Colorado Department of Corrections (CDOC) for respondents participating in a state procurement process. The document requires signatories to affirm compliance with Colorado Revised Statutes sections 18-8-302, 18-8-306, 18-8-301, and 18-8-308, as well as the State of Colorado State Controller Policy on Procurement Conflicts of Interest. Respondents must certify that they have no actual, potential, perceived, or apparent conflicts of interest and understand that the Procurement Official has authority to remove them from the process at any time. The document establishes that participants may have access to State information and records, which must not be distributed, sold, shared with third parties, or used in any manner except as expressly authorized.
The attestation requires authorized signatories to declare they have no personal or beneficial interest in the service or property described in the relevant Colorado Revised Statutes and commits them to notify the Procurement Agent immediately if any situation arises that could alter their representations or create an appearance of conflict or impropriety. Signatories must identify themselves as either employees of private employers or independent contractors under contract with the State of Colorado and acknowledge they are receiving such information in that capacity. The document must be signed and notarized with the printed name of the authorized signer and the company name or doing business as designation.
