About this file
This document is a Vendor Work Order Request form used by the State of North Dakota for procuring vendor services through a state contract. The form is a standardized template (SFN 54383) designed to facilitate the process of requesting and authorizing work orders between state agencies and vendors, with provisions for detailed project scoping, staffing requirements, and vendor selection.
The form outlines a structured approach for work order proposals, including evaluation criteria weighted across vendor project proposal (30 points), staff qualifications and availability (40 points), and cost (30 points). The document supports both Time and Materials and Fixed Bid project types, requiring vendors to submit detailed proposals that include project strategy, staff resumes, reference checks, and cost structures. Vendors must propose hourly service rates or fixed project prices that do not exceed rates originally proposed in the initial Request for Proposal (RFP), with provisions for potential multi-year rate considerations and mandatory background checks for contracted staff.
