Evaluate contractor performance for each evaluation criteria and provide ratings and/or comments as appropriate. Utilize the following guidelines:

After completion of individual categories, circle the summary rating evaluation provided at the end of this form for the composite Mission Closeout Report (MCR) evaluation.
Evaluation Criteria Comments The MCR shall include the following items that describe various performance elements. The accuracy and completeness of these items shall be the basis of evaluation for the MCR.
An executive summary describing the mission in general from MIC to MCR including mission results, significant problems that occurred throughout the mission and associated lessons learned, and new processes implemented to preclude future occurrences
A self assessment of NSROC performance for the mission
A final schedule reflecting actual major milestones for the mission including: MIC, RDM, DR, MRR, Field Ops., Launch, Failure/Anomaly Investigations, Shipping Milestones (to include outbound and return), Data Delivery, & MCR
A final listing of the payload team members assigned throughout the life of the mission (to include personnel changes)
A complete log of nonconformances & corrective actions associated with the mission including dates assigned & closed (all disciplines)
A description of all “Lessons Learned” on the mission (including software related) and declaration that they have been entered into the Lessons Learned Database
The final Requirements Compliance Matrix indicating if mission results achieved each requirement and concurred by SRPO
A detailed description of the field operations from MRR through Recovery including things that went well, things that did not, and a detailed schedule

Evaluation Criteria Comments
A comprehensive Risk Matrix for the mission that also identifies risks associated with any new or modified software – Risk Matrix is signed by the PI
A log listing procedures completed for assembly of the vehicle & payload (all disciplines)
A completed Safety Removal Checklist indicating all items retrieved, dates and initials of those persons providing the items along with any exceptions noted
A copy of the completed Mission Manager’s launch countdown procedure
A copy of all post flight reports including performance, GNC, telemetry, power, mechanical, and launch vehicles that document systems performance. Significant discrepancies between actual and predicted performance and flight parameters shall be investigated and resolved.
A tabulation that indicates which post flight reports (if any) address new/modified software utilized on the mission. Each post flight report shall include a dedicated section that summarizes any new/modified software (if any), any related issues encountered throughout the mission lifecycle, and an evaluation of the software’s actual inflight performance.
A statement from the Principal Investigator that all requested data products have been received (email is acceptable)
Any launch schedule delays that are attributable to NSROC functional areas of responsibility
Any NSROC initiated actions that exceed contract requirements
Satisfactory Mission Closeout Report received within 180 calendar days of launch date
