Trench Safety Act Requirement Form for Upper-Level Roadway Improvements, Phase 3 at Fort Lauderdale-Hollywood International Airport

This document is a Trench Safety Act Requirement form issued by Broward County, Florida, for the Upper-Level Roadway Improvements, Phase 3 project at Fort Lauderdale-Hollywood International Airport. The form must be completed and returned with the vendor's submittal, or submitted within three business days of the County's request; failure to comply with these timeframes may result in the vendor being deemed nonresponsive. The project involves essential bridge maintenance and structural repairs at the active airport facility, including the removal and replacement of bridge expansion joints, repair of concrete spalls, crack sealing, bridge jacking, and replacement of bridge bearings. Work is located along the upper and lower level roadway system in front of Terminals 2, 3, and 4, with critical requirements to maintain airport operations and ensure uninterrupted pedestrian and vehicular access throughout execution.

The Trench Safety Act, as incorporated into Florida Statutes (Sections 553.60–553.64), applies to any individual project with trenches exceeding 5 feet in depth and incorporates OSHA excavation safety standards (29 C.F.R. Section 1926.650). Vendors must affirm awareness of these requirements and identify the anticipated method and cost of compliance, including special shoring requirements if applicable. The form provides space for vendors to submit pricing and compliance methods using either an open-end contract format (with unit prices for linear feet of work and square feet of special shoring) or a fixed contract format (with quantity-based pricing). All costs and methods for achieving trench safety compliance are legally binding on the vendor, its employees, and all subcontractors involved in the project.
