About this file
This is a Criminal History Screening Practices Certification form required by Broward County, Florida for vendors seeking contracts valued at $100,000 or more. The certification pertains to the Upper-Level Roadway Improvements, Phase 3 project at Fort Lauderdale-Hollywood International Airport, which involves furnishing labor, materials, equipment, and services for essential bridge maintenance and structural repairs along the airport's upper and lower level roadway system in front of Terminals 2, 3, and 4. The scope of work includes removing and replacing bridge expansion joints, repairing concrete spalls, sealing cracks, executing a bridge jacking plan, and replacing bridge bearings while maintaining continuous airport operations and uninterrupted pedestrian and vehicular access. Vendors must submit the completed certification form with their solicitation response or within three business days of the County's request, with failure to comply potentially resulting in a nonresponsive determination.
The certification requires vendors to implement or commit to implementing policies and procedures that preclude inquiry into an applicant's criminal history until the applicant is selected as a finalist and interviewed for positions located within the United States that will foreseeably perform work under the Broward County contract. Vendors may be exempt from this requirement if they are required by applicable federal, state, or local law to conduct criminal history background checks at a different time or manner, or if the vendor is a governmental agency. Non-compliance with Section 26-125(d) of the Broward County Code of Ordinances constitutes a material breach and may result in contract termination, debarment, or suspension proceedings. The form is dated September 9, 2024, and requires vendor certification through signature, printed name, title, and date.
