About this file

This is an Attachment D evaluation document for a Utah State Cooperative Contract for Shelf Stable Foods issued by the State of Utah Division of Purchasing. The RFP seeks to establish a contract for the provision of shelf stable food items to the State of Utah, with the potential for multiple awards. The contract term is five (5) years, with vendor pricing guaranteed for three (3) months on a quarterly basis. Offerors must certify compliance with the Scope of Work (Attachment B), provide pricing for all line items on the Market Basket Cost Sheet (Attachment C) in the specified units of measure, and submit a completed cost sheet in the U3P-Bonfire system. The document does not specify response dates, due dates, site visits, or bidders' conferences.

The evaluation process combines technical and cost scoring, with technical criteria weighted at 70 percent (1,850 points possible, with a minimum threshold of 1,295 points required) and cost criteria weighted at 30 percent (792.9 points available). Technical scoring categories include Deliverables/Contract Requirements (500 points), Delivery Services (250 points), Ordering/Order Entry (250 points), Product Recall procedures (150 points), Disaster Plan Deliveries (150 points), Full Product Line submission (250 points), Experience and Qualifications (150 points), and Statewide Delivery capability (150 points). To qualify for award consideration, offerors must meet the technical minimum threshold of 1,295 points and achieve an overall score of 1,850 total points or higher for multiple award eligibility. Cost points are allocated using a formula in which the lowest-cost offeror receives the full 792.9 cost points, while all other offerors receive points proportional to their cost relative to the lowest bid. An administrative fee of 0.75 percent applies to all transactions under the contract.
