About this file
This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) under the State of Colorado, required in accordance with Colorado Revised Statutes 24-102-206. The form must be completed and returned by vendors as part of the bid submission process. It serves as official notice to the Department of Personnel when a contractor or subcontractor intends to perform services outside the United States or the State of Colorado. The form requires vendors to disclose whether any contract services or subcontracted work will be performed outside these jurisdictions, and if applicable, to identify the specific types of services, locations, and justification for performing work externally.
Vendors must complete four sections if services are to be performed outside the U.S. or Colorado: identifying whether services will be performed externally, describing the types of services (such as software, hosting, technical support, server monitoring, or design and development), specifying the countries or states where services will be performed, and explaining the necessity or business advantage of performing services outside these jurisdictions. The form requires authorization by the business president, board chairperson, or contract authorized signatory. Upon completion, vendors must return the form to the sole contact listed in the solicitation. The procuring agency records solicitation number, contract type, service type, and whether the vendor provided notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado.
