About this file

This is a Vendor's Statement form pursuant to Kentucky Revised Statute (KRS) 45A.343, required by the City of Owensboro, Kentucky for contractors bidding on the Digital Kiosks project. The document serves as a compliance certification in which vendors must affirm that they have not been finally determined to have violated any provisions of specified Kentucky statutes (KRS Chapters 136, 139, 141, 337, 338, 341, and 342) within the five-year period preceding submission. The form requires vendors to acknowledge their understanding that compliance with these statutory provisions is mandatory throughout the duration of any resulting contract with the City of Owensboro.

The statement outlines the consequences of nondisclosure or noncompliance with the referenced Kentucky statutes. Vendors must understand that failure to reveal any final violation determination or failure to maintain compliance during contract performance will authorize the City of Owensboro to cancel the contract and disqualify the vendor from eligibility for future city contracts for a period of two years. The form requires completion by a duly authorized officer of the contracting company, including signature, title, and submission date.
