About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for the City of Owensboro's Digital Kiosks project in Kentucky. The statement serves as a prerequisite for determining bidder responsibility and must be completed by all bidders or offerors submitting proposals for the project, which involves construction of three digital kiosks in Downtown Owensboro. The form requires the bidder's authorized representative to provide their office title, company name, and signature, with the statement dated in 2025.
The sworn statement requires bidders to affirm under penalty of perjury that neither they nor, to the best of their knowledge, anyone acting on their behalf has knowingly violated any campaign finance laws of the Commonwealth of Kentucky. Additionally, bidders must certify that awarding the contract to them will not violate any provision of Kentucky's campaign finance laws. The term "knowingly" is defined to mean the bidder is aware or should have been aware that their conduct or circumstances described violate applicable statutes. This compliance certification is a mandatory submission requirement for all bidders seeking consideration for the Digital Kiosks contract.
