About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico for the Fire Station 4 Replacement Project. The form serves as a certification requirement for bidders participating in this construction procurement. Bidders must certify their eligibility to participate in the project by confirming they are not debarred, suspended, proposed for debarment, or otherwise ineligible for covered transactions with any federal, state, or local agency. The form requires authorized signatures and must be submitted as part of the bidding process for the construction of a new Fire Station facility at 4401 Diamond Drive on vacant land west of the existing Fire Station 4 building.
The certification form requires bidders to disclose any convictions, civil judgments, or criminal charges within the preceding three years related to fraud, antitrust violations, embezzlement, theft, bribery, falsification of records, or other crimes indicating lack of business integrity. Bidders must also confirm they have not violated contract performance provisions, ethical standards under the County Procurement Code, or had public contracts terminated for cause or default within three years. Additionally, bidders must certify that neither they nor their immediate family members are current or former County employees or public officials. Any inability to certify these statements requires written explanation. Falsification of any certification statement constitutes grounds for non-consideration or rescission of a contract award. The contractor must also confirm that all proposed subcontractors are not currently suspended or debarred from conducting business with any governmental entities.
