GENERAL REQUIREMENTS – ST. CLOUD VA MEDICAL CENTER
1.1 SAFETY REQUIRMENTS
Refer to section 01 35 26, SAFETY REQUIREMENTS for safety and infection control requirements.
1.2 GENERAL INTENTION
A. Contractor shall completely prepare site for bui lding operations, including demolition and removal of existing struct ures, and furnish labor and materials and perform work for Remodeling Bldg. 1, Rm. 33, X- Ray room 1 for installation of a new RF system per VA specification and as required by drawings and specifications provided by the A/E.
B. Visits to the site by Bidders may be made only b y appointment with the Contracting Officer.
C. Offices of Leo A Daly, as Architect-Engineers, w ill render certain technical services during construction. Such servic es shall be considered as advisory to the Government and shall not be construed as expressing or implying a contractual act of the Gov ernment without affirmations by Contracting Officer or his duly aut horized representative.
D. Before placement and installation of work subjec t to tests by testing laboratory retained by the Contractor. The Contract or shall notify the Contracting Officer’s Representative (COR) not less than two weeks in advance and shall coordinate with testing laborator y personnel to provide sufficient time to enable testing laborator y personnel to be present at the site in time for proper taking and t esting of specimens and field inspection.
E. All employees of general contractor and subcontr actors shall comply with VA security management program and obtain perm ission of the VA police, be identified by project and employer, and restricted from unauthorized access.
F. Prior to commencing work, general contractor sha ll provide proof that the project supervisor having an OSHA 30 certified “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present.
G. Training:
1. The Contractor’s project supervisor is required to attend GEMS and
Safety training provided by VA St. Cloud. Training must be attended prior to being designated as a job supervisor on an y VA St. Cloud construction project.
2. All employees of general contractor or subcontra ctors shall have the at minimum the 10-hour OSHA certified Construction Safety course and other relevant competency training, as determined b y VA CP with input from the Infection Control Risk Assessment (I CRA) team.
3. Submit training records of all such employees fo r approval before the start of work.
4. Notice to proceed will be issued not less than 2 weeks after receipt of bonds; time extensions will not be granted becau se of the need for training.
H. Identification Badge:
All contractor employees working on this project w ill be required to obtain and wear while on VA property, a VA picture identification badge. The badge will only be issued to those empl oyees having the appropriate OSHA Construction Safety Cards. The ba dges must be requested 2 weeks in advance of working on site and will be issued free
01 00 00.02 -2 St. Cloud Site of charge by the VA. All ID badges must be returne d upon contract completion. There will be a $200 charge for each P IV/Flash ID badge not returned at the end of the contract. There wil l be a $25 charge for “facility” badges and “contractor” or consultan t badges
I. Project Acceptance (Substantial Completion):
1. The acceptance of a project for substantial comp letion is to include the following:
a. The completion of all items to meet the criteri a of the contract drawings and specifications to the satisfaction of the Contracting Officer (CO). Items for correction may be considered to be punch list items, as determined by the CO, if the COR finds them to be minor in correction. Value for the corre ctions will be held by the VA, as determined by the CO, until all corrections are completed to the satisfaction of the CO.
b. The VA will not accept a project, or phase of a project as determined by contract documents, as substantially complete until a complete passing test and balance report of the H VAC system has been submitted and accepted as complete and passing by the CO. It is recommended that the HVAC system be completed wi th sufficient time to make corrections to submit a passing report . A time extension to the contract will not be considered fo r corrections to the HVAC system that are determined by the CO to be installation or design errors if within the contrac t.
1.3 STATEMENT OF BID ITEM(S)
A. ITEM I, GENERAL CONSTRUCTION: Work includes gene ral construction, alterations and necessary removal of existing struc tures and construction and certain other items.
ITEM II, Electrical Work: Work includes all labor, material, equipment and supervision to perform the required electrical cons truction work on this project.
ITEM III, Mechanical Work: Work includes all labor, material, equipment and supervision to perform the required Mechanical construction work on this project.
1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR
A. AFTER AWARD OF CONTRACT, Contractor is to provid e his/her own drawings and specifications as downloaded from WWW.FBO.gov
1.5 CONSTRUCTION SECURITY REQUIREMENTS
A. Security Plan:
1. The security plan defines both physical and admi nistrative security procedures that will remain effective for the entir e duration of the project.
2. The General Contractor is responsible for assuri ng that all sub-contractors working on the project and their employ ees also comply with these regulations.
B. Security Procedures:
1. General Contractor’s employees shall not enter t he project site without appropriate badge. They may also be subjec t to inspection of their personal effects when entering or leaving the project site.
2. All contractor and subcontractor employees worki ng on this project are subject to a background investigation. VA has t he right to refuse to badge any employee that does not pass the background investigation. It is expected that the contractor w ill have the employee scheduled for the issuance of a badge well in advance of starting work. Due to the badge process, the employ ee will not be
01 00 00.02 -3 St. Cloud Site able come to the VA, receive badge, and conduct wor k on same day.
There will be a $200 fine for badges issued and not returned upon completion of project.
Contractor and subcontractor employees that will work on VA property shall submit the following information to the Cont racting Officer’s Representative (COR) when requesting a badge:
First, middle and last name (Legal name, as shown o n picture ID) Date of Birth (DOB) Social Security Number (SSN) Height Eye Color Hair Color Name of Firm or Company VA Contract Number VA Project Name Name of COR
3. Before starting work the General Contractor shal l give one week’s notice to the COR so that security arrangements can be provided for the employees. This notice is separate from any no tices required for utility shutdown described later in this sectio n.
4. For working outside the “regular hours” as defin ed in the contract, The General Contractor shall give 10 business day n otice to the Contracting Officer and the COR so that security ar rangements can be provided for the employees. This notice is separat e from any notices required for utility shutdown described lat er in this section.
5. No photography of VA premises is allowed without written permission of the Contracting Officer.
6. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the pr emises in the event of a national emergency. The General Contrac tor may return to the site only with the written approval of the Cont racting Officer.
D. Key Control:
1. Door hardware installed in construction doors is to be self-closing and storage function lock, able to receive a BEST 7 pin core and only operable with a key. The VA will install the c onstruction core and issue keys to the contractor’s personnel. All c onstruction fences are to be locked with a VA lock in series so VA engineering and police personnel have emergency access at all t imes.
Construction fences are to be kept locked at all ti mes to prevent access by patients and VA unauthorized staff. Contr actor is to provide means of egress from the site that keeps th e site secure from the exterior. Keys to necessary construction a reas can be checked out with the approval of the COR. The contr actor is to give a minimum of 10 business day notice for security ap proval for areas that need to be entered for construction purposes.
2. The General Contractor shall turn over all perma nent lock cylinders to the VA locksmith for permanent installation. See Section 08 71 00, DOOR HARDWARE and coordinate.
3. VA construction core keys will be issued to the contractor as deemed necessary by the COR. All keys must be returned wh en no longer needed or upon completion of the contract. There w ill be a $25 charge for each key not returned at the end of the contract. Should VA security be compromised as a result of failure t o return a
01 00 00.02 -4 St. Cloud Site key(s), there will be an additional charge to the c ontractor of $25 for each door re-cored.
E. Document Control:
1. Before starting any work, the General Contractor /Sub Contractors shall submit an electronic security memorandum desc ribing the approach to following goals and maintaining confide ntiality of “sensitive information”.
2. The General Contractor is responsible for safeke eping of all drawings, project manual and other project informat ion. This information shall be shared only with those with a specific need to accomplish the project.
3. Certain documents, sketches, videos or photograp hs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”.
Secure such information in separate containers and limit the access to only those who will need it for the project. Re turn the information to the Contracting Officer upon request .
4. These security documents shall not be removed or transmitted from the project site without the written approval of Co ntracting Officer.
5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.
6. Notify Contracting Officer and Site Security Off icer immediately when there is a loss or compromise of “sensitive in formation”.
7. All electronic information shall be stored in sp ecified location following VA standards and procedures using an Engi neering Document Management Software (EDMS).
a. Security, access and maintenance of all project drawings, both scanned and electronic shall be performed and track ed through the EDMS system.
b. “Sensitive information” including drawings and o ther documents may be attached to e-mail provided all VA encryptio n procedures are followed.
F. Motor Vehicle Restrictions
1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitt ed 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.
2. A limited number of (3) permits shall be issued for General Contractor and its employees for parking in designa ted areas only (contractor parking lot adjacent to the wind turbin e).
1.6 OPERATIONS AND STORAGE AREAS
A. The Contractor shall confine all operations (inc luding storage of materials) on Government premises to areas authoriz ed or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers and agents, free and harml ess from liability of any nature occasioned by the Contractor's perfor mance.
B. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the appr oval of the Contracting Officer and shall be built with labor a nd materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor a t its expense upon completion of the work. With the written consent of the Contracting Officer, the buildings and utilities may be abandon ed and need not be removed.

C. The Contractor shall, under regulations prescrib ed by the Contracting Officer, use only established roadways, or use temp orary roadways constructed by the Contractor when and as authorize d by the Contracting Officer. When materials are transported in prosecut ing the work, vehicles shall not be loaded beyond the loading cap acity recommended by the manufacturer of the vehicle or prescribed by an y Federal, State, or local law or regulation. When it is necessary to cr oss curbs or sidewalks, the Contractor shall protect them from d amage. The Contractor shall repair or pay for the repair of an y damaged curbs, sidewalks, or roads.
D. Working space and space available for storing ma terials shall be as shown on the drawings; within the construction site only.
E. Workmen are subject to rules of Health Care Syst em applicable to their conduct.
F. Execute work so as to interfere as little as possible with normal functioning of Health Care System as a whole, inclu ding operations of utility services, fire protection systems and any e xisting equipment, and with work being done by others. The Contractor shall notify the COR prior to the use of equipment and tools that transm it vibrations and noises that can be either felt or heard outside the work site (core drilling, chipping hammer, jack hammer etc.). COR a pproval to use such equipment and tools shall be obtained in advance, n ot less than 48 business hours prior to the use of such tools, in o rder to allow advance coordination with health care staff. Contra ctor to include pricing in the offer for executing this work off ho urs, after 4:30 pm or as indicated in the construction documents. This applies to all VA occupied space and any occupied space adjacent to c onstruction activities where noise or vibration can be felt or heard.
1. Do not store materials and equipment in other t han assigned areas.
2. Schedule delivery of materials and equipment to immediate construction working areas quantities sufficient fo r not more than one work day. Provide unobstructed access to Health Care System areas required to remain in operation.
3. Where access by Health Care System personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and saf ety requirements and will be coordinated in advance with the COR.
G. Utilities Services: Where necessary to cut exi sting pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (excep t telephone), they shall be cut and capped at suitable places where sh own in the project drawings. In absence of such indication, in the dra wings, prior to any cutting or capping, the Contractor shall coordinate cutting and capping with the COR. The Contractor shall also coordinate all such actions Utility Company involved:
1. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the c onstruction project, for such items as water, sewer, electricit y, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.
2. Contractor shall submit a request to interrupt any such services to COR, in writing, not less than one week in advance of proposed interruption. Request shall state reason, date, exa ct time of, and approximate duration of such interruption.

3. Contractor will be advised (in writing) of approval of request, or of which other date and/or time such interruption w ill cause least inconvenience to operations of Health Care System. Interruption time approved by Health Care System may occur at other t han Contractor's normal working hours.
4. Major interruptions of any system must be requested, in writing, at least 15 calendar days prior to the desired time an d shall be performed as directed by the COR.
5. In case of a contract construction emergen cy that service will be interrupted shall inform COR immediately. Such inte rruption and cause of interruption will be explained in writing within 72 hours.
H. Abandoned Lines: All service lines such as wir es, cables, conduits, ducts, pipes and the like, and their hangers or sup ports, which are to be abandoned but are not required to be entirely remov ed, shall be sealed, capped or plugged. The lines shall not be capped in finished areas, but shall be removed and sealed, capped or plugged in c eilings, within furred spaces, in unfinished areas, or within walls or par titions; so that they are completely behind the finished surfaces.
I. To minimize interference of construction activ ities with flow of Health Care System traffic, comply with the following:
1. Keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction m aterials, debris and standing construction equipment and vehicles.
J. Coordinate the work for this contract with oth er construction operations The Contractor shall inform the COR of all such coo rdination. This includes the scheduling of traffic and the use of r oadways, as specified in Article, USE OF ROADWAYS.
K. Phasing:
The Medical Center must maintain its operation 24 hours a day 7 days a week. Therefore, any interruption in service must be scheduled in advance and coordinated with the COR to ensure that no lapses in operation occur. It is the CONTRACTOR'S responsibi lity to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, t he interim life safety measure to be used during the work, and a sc hedule defining the duration of the work with milestone subtasks.
To insure such executions, Contractor shall furnis h the COR with a schedule of approximate phasing and dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. In addition, Contractor shall noti fy the COR two weeks in advance of the proposed date of starting work in each specific area of site, building or portion thereof. All phasing d ates shall be arranged to insure accomplishment of this work in s uccessive phases as detailed in the Construction Drawings for phasing.
L. Building No. 1 will be occupied during performan ce of work.
Contractor shall take all measures and provide all material necessary for protecting existing equipment and pro perty in affected areas of construction against dust and debris, so t hat equipment and affected areas to be used in the Medical Centers op erations will not be hindered. Contractor shall permit access to Depa rtment of Veterans Affairs personnel and patients through oth er construction
01 00 00.02 -7 St. Cloud Site areas which serve as routes of access to such affec ted areas and equipment. These routes whether access or egress s hall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and s taff access.
Coordinate alteration work in areas occupied by Dep artment of Veterans Affairs so that Medical Center operations will continue during the construction period.
2. Immediate areas of alterations not mentioned in preceding Subparagraph 1 will be temporarily vacated while al terations are performed.
M. Construction Fence: Should it be determined by the COR that an outside lay down area is required the following constructio n fence criteria will be met. Before construction operations begin, Contractor shall provide a chain link construction fence, 2.1m (seve n feet) minimum height, around the construction area indicated on t he drawings or as required confining all construction activities. Pro vide gates as required for access with necessary hardware, includ ing hasps and padlocks. Contractor must provide hardware on gate to provide exit ability of contractor’s staff and not allow access to patients at the facility. VA engineering staff must have the abilit y to access this gate at any time. Provide gates as required for acc ess with necessary hardware, including hasps and padlocks. Fasten fenc e fabric to terminal posts with tension bands and to line posts and top and bottom rails with tie wires spaced at maximum 375mm (15 inches). Bottom of fences shall extend to 25mm (one inch) above grade. Remova l of construction fence shall be coordinated in advance with the COR.
N. When a building and/or construction site is tur ned over to Contractor, Contractor shall accept entire responsibility inclu ding upkeep and maintenance therefore:
1. Contractor shall maintain a minimum temperature of 4 degrees C (40 degrees F) at all times, except as otherwise specif ied.
2. Contractor shall maintain in operating condition existing fire protection and alarm equipment. In connection with fire alarm equipment, Contractor shall make arrangements for p re-inspection of site with Fire Department or Company (Department of Veterans Affairs or municipal) whichever will be required to respond to an alarm from Contractor's employee or watchman.
O. Utilities Services: Maintain existing utility se rvices for Health Care System at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uni nterrupted services.
Where necessary to cut existing water, steam, gases , sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (incl uding telephone), they shall be cut and capped at suitable places whe re shown; or, in absence of such indication, the Contractor shall co ordinate in advance with the COR and receive COR approval to proceed pr ior to any such cuts or caps.
1. No utility service such as water, gas, steam, se wers or electricity, or fire protection systems and communications syste ms may be interrupted without prior approval of the COR. No “ HOT TAPPING” of any utility service other than storm or sanitary ut ilities is allowed unless under extreme circumstances. If thes e circumstances are determined appropriate and approved by the COR, all work must follow Facilities Management Memorandum 23 “Hot Tap ping Procedures”
All services under work shall be isolated and al l energy released before work begins. Electrical work shall be accomp lished with all
01 00 00.02 -8 St. Cloud Site affected circuits or equipment de-energized. When a n electrical outage cannot be accomplished, work on any energize d circuits or equipment shall not commence without a detailed wor k plan, the Medical Center Director’s prior knowledge and writt en approval.

ELECTRICAL INSTALLATIONS, 27 05 11 REQUIREMENTS FOR COMMUNICATIONS
INSTALLATIONS and 28 05 11, REQUIREMENTS FOR ELECTR ONIC SAFETY AND SECURITY INSTALLATIONS for additional requirements.
2. Contractor shall submit a request to interrupt a ny such services to the COR, in writing, 7 days in advance of proposed interruption.
Request shall state reason, date, exact time of, an d approximate duration of such interruption.
3. Contractor will be advised (in writing) of appro val of request, or of which other date and/or time such interruption w ill cause least inconvenience to operations of Health Care System. Interruption time approved by Medical Center may occur at other than Contractor's normal working hours.
4. Major interruptions of any system must be reques ted, in writing, at least 15 calendar days prior to the desired time an d shall be performed as directed by the COR.
5. In case of a contract construction emergency whi ch interrupts service will notify the COR immediately. Such inter ruption and explanation will be confirmed in writing within 72 hours of the incident.
6. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the c onstruction project, for such items as water, sewer, electricit y, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.
P. Abandoned Lines: All service lines such as wires , cables, conduits, ducts, pipes and the like, and their hangers or sup ports, which are to be abandoned but are not required to be entirely re moved, shall be sealed, capped or plugged at the main, branch or pa nel they originate from. The lines shall not be capped in finished are as, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or par titions; so that they are completely behind the finished surfaces.
Q. To minimize interference of construction activit ies with flow of Medical Center traffic, comply with the following:
1. Keep roads, walks and entrances to grounds, to p arking and to occupied areas of buildings clear of construction m aterials, debris and standing construction equipment and vehicles. W herever excavation for new utility lines cross existing roa ds, at least one lane must be open to traffic at all times with appr oval.
2. The Contractor shall submit proposed methods and scheduling of required cutting, altering and removal of existing roads, walks and entrances to the COR not less than 7 work days in a dvance of any such work. Plans for such work must be approved in advance by the
COR.
R. Coordinate the work for this contract with other construction operations and notify the COR in advance of schedul ing of traffic and the use of roadways, as
1.7 ALTERATIONS
A. Survey: Before any work is started, the Contract or shall make a thorough survey with the COR and a representative o f VA Supply Service, 01 00 00.02 -9 St. Cloud Site of buildings areas of buildings in which alteration s occur and areas which are anticipated routes of access, and furnish a report, signed by both all three to the Contracting Officer. This rep ort shall list by rooms and spaces:
1. Existing condition and types of resilient floori ng, doors, windows, walls and other surfaces not required to be altered throughout affected areas of buildings.
2. Existence and conditions of items such as plumbi ng fixtures and accessories, electrical fixtures, equipment, veneti an blinds, shades, etc., required by drawings to be either reu sed or relocated, or both.
3. Shall note any discrepancies between drawings an d existing conditions at site.
4. Shall designate areas for working space, materia ls storage and routes of access to areas within buildings where al terations occur and which have been agreed upon by Contractor and t he COR.
B. Any items required by drawings to be either reus ed or relocated or both, found during this survey to be nonexistent, o r in opinion of the COR, to be in such condition that their use is impo ssible or impractical, shall be furnished and/or replaced by Contractor with new items in accordance with specifications which will be furnished by Government. Provided the contract work is changed b y reason of this subparagraph B, the contract will be modified accor dingly, under provisions of clause entitled "DIFFERING SITE CONDI TIONS" (FAR 52.236-
2) and "CHANGES" (FAR 52.243-4 and VAAR 852.236-88) .
C. Re-Survey: Thirty days before expected partial o r final inspection date, the Contractor and the COR together shall mak e a thorough re-survey of the areas of buildings involved. They sha ll furnish a report on conditions then existing, of resilient flooring, doors, windows, walls and other surfaces as compared with condition s of same as noted in first condition survey report:
1. Re-survey report shall also list any damage caus ed by Contractor to such flooring and other surfaces, despite protectio n measures; and, will form basis for determining extent of repair wo rk required of Contractor to restore damage caused by Contractor's workmen in executing work of this contract.
D. Protection: Provide the following protective mea sures:
1. Wherever existing roof surfaces are disturbed th ey shall be protected against water infiltration. In case of le aks, they shall be repaired immediately upon discovery.
2. Temporary protection against damage for portions of existing structures and grounds where work is to be done, ma terials handled and equipment moved and/or relocated.
3. Protection of interior of existing structures at all times, from damage, dust and weather inclemency. Wherever work is performed, floor surfaces that are to remain in place shall be adequately protected prior to starting work, and this protecti on shall be maintained intact until all work in the area is com pleted.
1.8 INFECTION PREVENTION MEASURES
A. Implement the requirements of VA’s Infection Contro l Risk Assessment
(ICRA) team. ICRA Group may monitor dust in the vi cinity of the construction work and require the Contractor to tak e corrective action immediately if the safe levels are exceeded.

B. Establish and maintain a dust control program as pa rt of the contractor’s infection preventive measures in accor dance with the guidelines provided by ICRA Group. Prior to start of work, prepare a plan detailing project-specific dust protection mea sures, including periodic status reports, and submit to Project Mana ger for review for compliance with contract requirements in accordance with Section 01 33
23, SHOP DRAWINGS, PRODUCT DATA SAMPLES.
1. All personnel involved in the construction or renov ation activity shall be educated and trained in infection preventi on measures established by the medical center.
C. Medical center Infection Control personnel shall mo nitor for airborne disease (e.g. aspergillosis) as appropriate during construction. A baseline of conditions may be established by the me dical center prior to the start of work and periodically during the co nstruction stage to determine impact of construction activities on indo or air quality. In addition:
1. The COR and VAMC Infection Control personnel shall review pressure differential monitoring documentation to v erify that pressure differentials in the construction zone and in the patient-care rooms are appropriate for their settin gs. The requirement for negative air pressure in the constr uction zone shall depend on the location and type of activity. Upon notification, the contractor shall implement correc tive measures to restore proper pressure differentials as needed.
2. In case of any problem, the medical center, along w ith assistance from the contractor, shall conduct an environmental assessment to find and eliminate the source.
D. In general, the following preventive measures shall be adopted during construction to keep down dust and prevent mold.
1. Dampen debris to keep down dust and provide tempora ry construction partitions in existing structures wher e directed by
Project Manager. Blank off ducts and diffusers to prevent circulation of dust into occupied areas during cons truction.
2. Do not perform dust producing tasks within occupied areas without the approval of the Project Manager. For construct ion in any areas that will remain jointly occupied by the medi cal Center and
Contractor’s workers, the Contractor shall:

a. Provide dust proof fire-rated temporary drywall con struction barriers to completely separate construction from t he operational areas of the hospital in order to conta in dirt debris and dust. Barriers shall be sealed and made presentable on hospital occupied side. Install a s elf-closing rated door in a metal frame, commensurate with the partition, to allow worker access. Maintain negative air at al l times. A fire retardant polystyrene, 6-mil thick or greater plastic barrier meeting local fire codes may be used where dust control is the only hazard, and an agreement is rea ched with the Project Manager and Medical Center.
b. HEPA filtration is required where the exhaust dust may reenter the breathing zone. Contractor shall verify that c onstruction exhaust to exterior is not reintroduced to the medi cal center through intake vents, or building openings. Instal l HEPA
(High Efficiency Particulate Accumulator) filter va cuum system rated at 95% capture of 0.3 microns including polle n, mold spores and dust particles. Insure continuous negat ive air pressures occurring within the work area. HEPA fil ters should have ASHRAE 85 or other prefilter to extend the use ful life of the HEPA. Provide both primary and secondary filtr ations units. Exhaust hoses shall be heavy duty, flexible steel reinforced and exhausted so that dust is not reintr oduced to the medical center.
c. Adhesive Walk-off/Carpet Walk-off Mats, minimum 600 mm x 900mm
(24” x 36”), shall be used at all interior transiti ons from the construction area to occupied medical center ar ea. These mats shall be changed as often as required to maint ain clean work areas directly outside construction area at al l times.
d. Vacuum and wet mop all transition areas from constr uction to the occupied medical center at the end of each work day.
Vacuum shall utilize HEPA filtration. Maintain surr ounding area frequently. Remove debris as they are created .
Transport these outside the construction area in co ntainers with tightly fitting lids.
e. The contractor shall not haul debris through patien t-care areas without prior approval of the Project Manager and the

Medical Center. When, approved, debris shall be ha uled in enclosed dust proof containers or wrapped in plasti c and sealed with duct tape. No sharp objects should be allowed to cut through the plastic. Wipe down the exterior of the containers with a damp rag to remove dust. All equ ipment, tools, material, etc. transported through occupied areas shall be made free from dust and moisture by vacuuming an d wipe down.
f. Using a HEPA vacuum, clean inside the barrier and v acuum ceiling tile prior to replacement. Any ceiling acc ess panels opened for investigation beyond sealed areas shall be sealed immediately when unattended.
g. There shall be no standing water during constructio n. This includes water in equipment drip pans and open cont ainers within the construction areas. All accidental spil ls must be cleaned up and dried within 12 hours. Remove and d ispose of porous materials that remain damp for more than 72 hours.
h. At completion, remove construction barriers and cei ling protection carefully, outside of normal work hours. Vacuum and clean all surfaces free of dust after the remov al.
E. Final Cleanup:
1. Upon completion of project, or as work progresses, remove all construction debris from above ceiling, vertical sh afts and utility chases that have been part of the construct ion.
2. Perform HEPA vacuum cleaning of all surfaces in the construction area. This includes walls, ceilings, cabinets, fur niture (built-in or free standing), partitions, flooring, etc.
3. All new air ducts shall be cleaned prior to final i nspection.
1.9 DISPOSAL AND RETENTION
A. Materials and equipment accruing from work remov ed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:
1. Reserved items which are to remain property of t he Government are identified by attached tags or noted on drawings or in specifications as items to be stored. Items that re main property of the Government shall be removed or dislodged from p resent locations in such a manner as to prevent damage which would b e detrimental to re-installation and reuse. Store such items where d irected by the
COR.
2. Items not reserved shall become property of the Contractor and be removed by Contractor from Health Care System.

3. Items of portable equipment and furnishings loca ted in rooms and spaces in which work is to be done under this contr act shall remain the property of the Government. When rooms and spac es are vacated by the Department of Veterans Affairs during the alter ation period, such items which are NOT required by drawings and s pecifications to be either relocated or reused will be removed by th e Government in advance of work to avoid interfering with Contracto r's operation.
4. PCB Transformers and Capacitors: The Contractor shall be responsible for disposal of the Polychlorinated Biphenyl (PCB) transformers and capacitors. The transformers and capacitors shall b e taken out of service and handled in accordance with the procedur es of the Environmental Protection Agency (EPA) and the Depar tment of Transportation (DOT) as outlined in Code of Federal Regulation (CFR), Titled 40 and 49 respectively. The EPA's Tox ic Substance Control Act (TSCA) Compliance Program Policy Nos. 6 -PCB-6 and 6-PCB- 7 also apply. Upon removal of PCB transformers and capacitors for disposal, the "originator" copy of the Uniform Haza rdous Waste Manifest (EPA Form 8700-22), along with the Uniform Hazardous Waste Manifest Continuation Sheet (EPA Form 8700-22A) sha ll be returned to the Contracting Officer who will annotate the contr act file and transmit the Manifest to the Medical Center's Cemet ery's Chief.

1.10 PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND
IMPROVEMENTS
A. The Contractor shall preserve and protect all st ructures, equipment, and vegetation (such as trees, shrubs, and grass) o n or adjacent to the work site, which are not to be removed and which do not unreasonably interfere with the work required under this contrac t. The Contractor shall only remove trees when specifically authorize d to do so, and shall avoid damaging vegetation that will remain in place. If any limbs or branches of trees are broken during contract per formance, or by the careless operation of equipment, or by workmen, the Contractor shall trim those limbs or branches with a clean cut and p aint the cut with a tree-pruning compound. Any grass that is damaged du ring construction will have the pre-existing grade restored, be sodde d and maintained until the sod is firmly rooted as determined by the COR. Sod will be
01 00 00.02 -14 St. Cloud Site watered by contractor and may not exceed 6 inches w hile the contractor is responsible.
B. The Contractor shall protect from damage all exi sting improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any dam age to those facilities, including those that are the property o f a third party, resulting from failure to comply with the requireme nts of this contract or failure to exercise reasonable care in performin g the work. If the Contractor fails or refuses to repair the damage pr omptly, the Contracting Officer may have the necessary work per formed and charge the cost to the Contractor.

C. Refer to Section 01 57 19, TEMPORARY ENVIRONMENT AL CONTROLS, for additional requirements on protecting vegetation, s oils and the environment. Refer to Articles, "Alterations", "Res toration", and "Operations and Storage Areas" for additional instr uctions concerning repair of damage to structures and site improvement s. At a minimum, the contractor is to comply with all EPA regulations fo r protection from storm water pollution that would be caused by const ruction and implement all required safeties to maintain complia nce. Also, all wash downs for concrete trucks is to be conducted off si te. No containment areas are allowed on site.
D. Refer to FAR clause 52.236-7, "Permits and Respo nsibilities," which is included in General Conditions. A National Pollutan t Discharge Elimination System (NPDES) and/or Storm Water pollu tion Prevention Plan (SWPPP) permit is required for this project. The Co ntractor is considered an "operator" under the permit and has e xtensive responsibility for compliance with permit requireme nts. VA will provide the permit application upon request available at th e (appropriate medical center) office. The Contractor and affected subcontractors shall furnish all information and certifications th at are required to comply with the permit process and permit requireme nts. Many of the permit requirements will be satisfied by completing construction as shown and specified. Some requirements involve the Contractor's method of operations and operations planning and the Contr actor is responsible for employing best management practices. The affect ed activities often include, but are not limited to the following:
- Designating areas for equipment maintenance and r epair;
- Providing waste receptacles at convenient locatio ns and provide regular collection of wastes;
- Locating equipment wash down areas on site, and p rovide appropriate control of wash-waters; concrete truck wash down is not allowed on site.
- Providing protected storage areas for chemicals, paints, solvents, fertilizers, and other potentially toxic materials; and
- Providing adequately maintained sanitary faciliti es.
1.11 RESTORATION
A. Remove, cut, alter, replace, patch and repair ex isting work as necessary to install new work. Except as shown in t he drawings or specified, do not cut, alter or remove any structur al work, and do not disturb any ducts, plumbing, steam, gas, or electri c work without prior written approval of the CO. Existing work to be alt ered or extended and that is found to be defective in any way, shall be reported to the COR before it is disturbed. Materials and workmanship u sed in restoring
01 00 00.02 -15 St. Cloud Site work, shall conform in type and quality to that of original existing construction, except as otherwise shown or specifie d.
B. Upon completion of contract, deliver work comple te and undamaged.
Existing work (walls, ceilings, partitions, floors, mechanical and electrical work, lawns, paving, roads, walks, etc.) disturbed or removed as a result of performing required new work , shall be patched, repaired, reinstalled, or replaced with new work, a nd refinished and left in as good condition as existed before commenc ing work.
C. At Contractor's own expense, Contractor shall im mediately restore to service and repair any damage caused by Contractor' s workmen to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications sy stems (including telephone) which are not scheduled for discontinuan ce or abandonment.
D. Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered b y adjustment to contract time and price in accordance with clause e ntitled "CHANGES" (FAR 52.243-4 and VAAR 852.236-88) and "DIFFERING S ITE CONDITIONS" (FAR 52.236-2).
