AMENDMENT 00003

08/16/2018

PROJECT:676-18-201 Build Whole Health
LOCATION:Tomah VA Medical Center in Tomah, WI

Amendment 00003 hereby incorporates the following change(s)/clarifications into the solicitation package:

A.) VENDOR QUESTION(S), 7/30/2018

1. Is there a nurse call system in this buildout? I did not see a spec if there is. The nurse call was just mentioned a couple of times.

VA REPLY: Plan Sheet TK-102 keyed notes (#4 and #5) indicate these as 120V dome lights and related pull switches. Any required power would be from adjacent receptacle circuits.

There is no central NC panel or annunciator.

B.) VENDOR QUESTION(S), 7/31/2018

1. There are (2) automatic door operators on single use restrooms (20748A & 20762A). These doors will require "push to lock" buttons, indicator switches and door position switches in order to provide privacy. Please confirm if this is required or intended.

VA REPLY: Yes, “push to lock”, indicator switches and door position switches are required.

C.) VENDOR QUESTION(S), 8/15/2018

1. We are a TAB company in La Crosse. I am looking at the documents for the VA build Whole Health Complex. Some of the HVAC spec’s reference TAB section 23 05 93 but that spec section is not included. Will this job require TAB work?

VA REPLY: No TAB is required for this project.

D.) VENDOR QUESTION(S), 08/15/2018

1. NP1-3 Has breakers listed for heat trace. The Mechanical specs talk about freeze protection on Hydronic outdoor piping. Do the Breakers need to be GFCI for equipment? Is there a plan showing the connection points for the exterior hydronic piping that needs freeze protection? VA REPLY: Notations in the panel schedule are a reflection of the existing conditions and previous heat trace systems serviced from the panel. There are no new heat trace systems or connections in this project. These are existing circuits and circuit designations.

E. VENDOR QUESTION(S), 08/16/2018

1. Can the superintendent, site safety & health officer and the contractor quality control staff be the same individual?

VA REPLY: Yes, but individual must always be on-site or have an APPROVED equal qualified alternate when not present.

2. Section 01 00 00-1.23-A calls for space on the buildings ground floor to be used for storage of equipment and materials. Approximately, how much square feet of space will be needed?

VA REPLY: The VA will not require space on the ground level. Delete 01 00 00-1.23.

3. How many units and locations for the negative air isolation monitoring stations would the resident engineer prefer?

VA REPLY: There shall be a sufficient number of machines to maintain negative pressure throughout the whole construction area for the full duration on the contract.

4. Is a pre-placement tuberculin screening required before being allowed to work on site?

VA REPLY: Yes. In accordance with Spec Section 01 35 26, Para 1.12, this is for contractor and patient protection.

F. VENDOR QUESTION(S), 8/16/2018

1. EP601 has a note (1) next to panel NP2-2. Note one says to replace that panel but there is no panel schedule to go with the panel. On the same one line panel NP3-2 is does not show a note (1) to replace. But on EP602 there is a panel schedule listed for NP3-2 with the rest of the panels being replaced. Can you please clarify if one or both of the panels are being replaced? Please clarify if these panels require the extra gutter on the side of them for splicing? Please clarify if the coordination study applies to what is being replaced or the entire electrical system? Please clarify what the manufacture of the main switch gear in the building?

VA REPLY:

· Panel NP2-2 is noted on EP601 and EP-102 as to be replaced. No new loads were added to this panel. Panel schedule is attached to this amendment (match existing). Ref A0003, Att1.

· Panel NP3-2 is noted on EP-102 to be replaced, a schedule is provided. Also, Drawing EP-601 applies. See sheet notes, in regards, to line weights and/or works scope.

· There is no requirement for added gutters on panels, this is a contractor choice/option.

· Sheet EP-601 notes: Arc flash study of the revised electrical system, this includes the entire system, not just the new equipment.

· The existing switchgear at the facility is Square D.

2. Please clarify if the existing electrical conduit for the fire alarm system need to move to above the drop ceiling or will the ceiling be lower to conceal the conduit? The ceil(ing) heights that the plans show in the hallway will mean that a lot of the fire alarm will have to move.

VA REPLY: Ceiling heights at the corridors shall be as indicated on the reflected ceiling plan, fire alarm drawings indicate to relocate devices to new ceiling. This may require relocating the raceways.

G. VENDOR QUESTION(S), 8/16/2018

1. Reference Demo page AD001: Walls showed dashed are to be demolished. Q: Are the walls to be taken out complete to upper decking, or above existing ceiling height.

VA REPLY: take demolished walls out completely, to deck.

H. VENDOR QUESTION, 8/16/2018

1. Need clarification on the following: Ref. Sheet AE-101, Key Note #5 Ballet Bar. There are no details on the prints, nor any listed spec section call out. Please provide detail and specs for material type, wall supports and anchoring requirements.

VA REPLY: See Amendment A00004, Attachment 2 for mounting details.

I. VENDOR QUESTION(S), 08/20/2018

1. Please clarify missing spec sections and if they are required. Table of contents shows sections for 23 05 41 (Noise & Vibration Control), 23 05 93 (Test & Balance) and 23 09 23(DDC). However, we do not see them in the actual specifications. Please clarify if these spec sections are required for the project.

VA REPLY: Specification sections 23 05 41 (Noise & Vibration Control), 23 05 93 (Test & Balance) and 23 09 23(DDC) are not used in this project. Delete from the Table of Contents.
