EXHIBIT F-1 SEED PROJECT
NOTICE OF PROPOSED PROJECT, PROPOSAL FORM

PROJECT DESCRIPTION: The Contractor shall provide all labor, materials, equipment, transportation, disposal and supervision necessary to replace the existing PACU air handling unit.
CAT I $2,000 - $1,000,000
MAGNITUDE OF PROJECT: Between $500,000 and $1,000,000
PERFORMANCE PERIOD: 365 Calendar Days
SITE VISIT: A site visit for this project will be conducted on 5/22/18 at 11am.
PERFORMANCE & PAYMENT BONDS: In accordance with FAR 28.102-1 and Contract Clause 52.228-15, Contractors are reminded that any amount awarded over $35,000 shall require Payment Bonds, and awards exceeding $150,000 shall require both Payment and Performance Bonds. All bonds are due no later than 10 days after award.
WAGE DETERMINATION: Currently, General Decision Number: MA180001 Modification No 2 dated 04/06/2018 is applicable to this task order. However, the current prevailing Wage Determination will be incorporated at time of award.
PROJECT SAFETY: In accordance with FAR 52.236-13, Accident Prevention, construction contractors shall immediately notify the Contracting Officer’s Representative (COR) and shall prepare a Report of Accident (VA Form 2162 or equivalent) in quadruplicate and forward the original and two copies to the Contracting Officer or COR for forwarding to Safety Officer.
PROPOSAL DUE DATE: All offers are due with proposal for MATOC basic contract. Failure to provide a price will make offer noncompliant, eliminated from the competition and offeror ineligible for a basic contract award. Basis for award is in the RFP.

0001 GENERAL CONSTRUCTION: Work includes the necessary supervision, labor, equipment, material, transportation, testing and infrastructure required for the contractor to replace the existing PACU air handling unit at the West Roxbury Veterans Affairs Medical Center, MA, in accordance with the Contract Documents, including Technical Specifications and Drawings. Work includes, but is not limited to, The work includes the removal of an existing PACU air handling unit and installation of a new PACU air handling unit including all services to the unit such as steam, heating and cooling water, electricity and communications. demolition, general construction, alterations, structural steel, roofing, electrical work and mechanical work for the removal of the existing PACU HVAC unit and the installation of a new PACU HVAC unit on the third floor roof of Building 1,section B, VAMC West Roxbury, MA. A general description of the work is as follows:
1. The PACU unit is in use between the hours of 6:00 AM and 6:00 PM Monday thru
Friday and will remain in operation during construction.
2. Provide temporary heating and cooling as required to maintain PACU use.
3. Provide a crane for removal of demolished equipment and installation of new equipment.
4. Remove the existing exterior roof mounted HVAC unit as well as related ductwork, piping and direct expansion condenser. Remove related steam heating and humidification systems. Remove related electrical services. Disconnect existing fire alarm devices.
5. Provide a new roof top mounted PACU HVAC unit at the same location as the original unit and reuse existing structural supports. Provide new structural steel as required to support new equipment from existing supports.
6. Provide new heating and cooling heat exchangers as well as related piping, connections and pumps. Provide new duct work and insulation.
7. Provide new unit controls and interface to existing Trane Tracer building automation system (BAS). Update BAS graphics to show new equipment and all related

8. Provide new clean steam humidification system.
9. Provide new electrical services for new HVAC equipment. Reconnect existing fire alarm devices.
10. Provide a complete air balance of the HVAC unit and the existing PACU HVAC system.
0002 ALTERNATE NO. 1: This alternate shall be to the BASE ITEM offer. The alternate includes all work required to accomplish the following general description:
1. Remove six duct cross connections and related dampers as shown on the drawings.
Remove and replace existing ceilings as required by the work.
0003 ALTERNATE NO. 2: This alternate shall be to the BASE ITEM offer. The alternate includes all work required to accomplish the following general description:
1. Replace existing reheat coil valves with new valves for six existing constant volume terminal units located above the ceiling in the PACU space.
2. Provide new duct temperature and space temperature sensors for the units.
3. Integrate the new sensors and controllers into the existing Trane Tracer BAS system.
Provide graphics for new components and show integration with new HVAC unit.
4. Rebalance reheat coils in the PACU space.
0004 ALTERNATE NO. 3: This alternate shall be to the BASE ITEM offer. The alternate includes all work required to accomplish the following general description:
1. Replace the existing exterior PACU isolation room exhaust fan with a new unit with variable speed drive.
2. Provide a new exterior filter housing package.
3. Integrate the new exhaust fan drive and the filter housing sensors and controllers into the existing Trane Tracer BAS system. Provide graphics for new components and show integration with new HVAC unit.
0005 Asbestos Sample Unit Price: This UNIT PRICE shall be the cost to sample and analyze a single location for asbestos containing materials including protective equipment, enclosures and testing: assume two (2) samples at each location per day. If more than one location is sampled in one day or if more than two samples are required, the cost of work shall be negotiated to recognize economy of scale and reduced costs for performing more than one unit per day.
0006 Asbestos Abatement Unit Price: This UNIT PRICE shall be the cost to remove less than three (3) square feet of asbestos containing material utilizing the OSHA glove bag method.
Provide $/SF cost including all equipment, materials, labor and disposal of material. If more than one location is abated in one day, the cost of work shall be negotiated to recognize economy of scale and reduced costs for performing more than one unit per day.

Use a format similar to the following to identify and calculate cost of the work to be self-performed. Refer to the definitions pertaining to “Self-performance of work”, “On the Site” and “Total amount of work to be performed under the contract”. (Includes mobilization and utilization of owned or rented plant and equipment to be operated by the prime contractor’s own employees; only those materials which will be both purchased and installed by the prime’s own forces; labor associated with those aforementioned materials or equipment; only those supplies to directly support work performed by the contractor’s own employees; and the contractor’s own job overhead costs.) Statement Of Work Construction
1. OVERVIEW
1.1 Government Agency Department and Address:

Project Title: SPD Exaust PACU HVAC Project Number: 523A4-12-211
2. DESCRIPTION OF WORK
2.1 General: Provide all labor, materials, equipment, transportation, supervision and disposal necessary for Project 523A4-12-211 SPD Exhaust PACU HVAC at the VA Boston HealthCare System, West Roxbury, MA Campus in accordance with the Statement of Work, Technical Specifications and Drawings.
PACU Air Handling Unit The existing PACU air handling unit (AHU), installed in 2003, is configured for Direct Expansion (DX) cooling and plant steam heating and humidification. This unit provides dedicated air handling for the PACU.
Due to high operational and maintenance costs and deviations from the current VA energy conservation initiatives and ASHRAE 90.1-2007, the existing unit is being replaced. The new unit shall have the same airflow and cooling/heating capacity, but shall include the following modifications to address the issues of cost and energy of the current system:
Direct drive fans to eliminate belts in the unit.
Glycol hot water coil and piping loop served by a shell and tube heat exchanger in Mechanical Room 3B-109 which utilizes plant steam to heat the glycol loop.
Glycol chilled water coil and piping loop served by a plate and frame heat exchanger in Mechanical Room 3B-109 which utilizes plant chilled water to cool the glycol loop.
A clean steam generator located inside the conditioned building.
Auxiliary connections to plant chilled water for emergency operation which bypass the heat exchange and pumps to directly connect the unit chilled water coil to the plant chilled water.
Variable frequency drives (VFDs) for supply and return fans.
Airflow measuring stations integrated with the existing Building Automation System.
Economizer controls and night/weekend setback scheduling.
Isolation Room Exhaust Fan (Alternate) The existing PACU Isolation Room exhaust fan runs at constant speed with no ability to adjust air flow.
The existing configuration has limited value when air balancing adjustments are required to maintain current VA HVAC criteria.
This design alternate provides for:
New exhaust fan with variable frequency drive (VFD).
New filter housing package.
Exhaust system monitoring integrated with the existing Building Automation System.
PACU Temperature Control (Alternate) It has been noted by the VA that temperatures within the PACU area can create uncomfortable conditions for patients. To address this, we have provided a design alternate controls upgrade for the space consisting of the following:
New constant volume terminal unit reheat coil valves and controllers.
New electronic space temperature sensors.
New electronic duct temperature sensors.
Rebalancing of reheat coils in the PACU space.
Integration of new sensors and controllers into the existing Building Automation System.

BUILDING CONSTRUCTION PROJECTS (does not include single family homes and apartments up to and including 4 stories) Note: Under Executive Order (EO) 13658, an hourly minimum wage of $10.35 for calendar year 2018 applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2015. If this contract is covered by the EO, the contractor must pay all workers in any classification listed on this wage determination at least $10.35 per hour (or the applicable wage rate listed on this wage determination, if it is higher) for all hours spent performing on the contract in calendar year 2018. The EO minimum wage rate will be adjusted annually. The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications;
however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014.
UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)
and 3.) should be followed.

GENERAL REQUIREMENTS
1.1 SAFETY REQUIREMENTS
Refer to section 01 35 26, SAFETY REQUIREMENTS for safety and infection control requirements.
1.2 GENERAL INTENTION
A. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for the PACU HVAC System
Improvement at the VA Medical Center, West Roxbury, Massachusetts, as required by drawings and specifications.
B. Visits to the site by Bidders may be made only by appointment with the
Medical Center Contracting Officer Representative (COR).
C. Offices of AI Engineers, Inc. 919 Middle Street, Middletown CT 06457, as Architect/Engineers, will render certain technical services during construction. Such services shall be considered as advisory to the
Government and shall not be construed as expressing or implying a contractual act of the Government without affirmations by Contracting
Officer or his duly authorized representative.
D. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access.
1.3 STATEMENT OF BID ITEM(S)
A. The Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for the project. The work includes the removal of an existing PACU air handling unit and installation of a new PACU air handling unit including all services to the unit such as steam, heating and cooling water, electricity and communications.
