About this file

This is a Proposer's General Questionnaire template for the University of Texas Health Science Center at San Antonio (UTHSA) related to the SABCS Temporary Security Staffing contract opportunity (RFP 745-26-P40). The University seeks qualified proposers to deliver comprehensive, integrated event security services including access control and credential verification, perimeter and door monitoring, emergency preparedness and incident response planning, deployment and management of staff communication systems, and coordination and oversight of on-site first aid and medical services. Proposers must demonstrate the capacity to scale staffing and operations in response to increasing attendance, evolving risk conditions, and event complexity. The questionnaire requires proposers to provide detailed information about their company profile, including legal name, business address, years in operation, state of incorporation, number of employees, annual revenue volume, and parent corporation details if applicable. Proposers must also submit a work plan with key dates and milestones, identification of tasks, timeframes, project management methodology, implementation strategy, and expected service implementation timeframe.

The questionnaire mandates that proposers submit customer reference lists comprising at least three organizations with which they currently have contracts or have previously provided similar services within the past five years, including company name, contact person information, project description, length of business relationship, and background of services provided. Additionally, proposers must provide summary resumes for key personnel, describe their service support philosophy and quality assurance programs, detail any anticipated difficulties and mitigation strategies, and disclose any relationships with University employees. Proposers must also indicate whether they will provide cloud computing services and submit contingency or disaster recovery plans if applicable. Financial documentation, litigation history, loan default status, and ownership information for individuals with at least 25% ownership interest may be requested. The UTHSA does not solicit or guarantee any specific amount of compensation, volume, minimum, or maximum amount of services under this RFP or resulting contract.
